Extracorporeal Stimulation of Sacral Nerve Roots for Observation of Pelvic Autonomic Nerve Integrity: Description of a Novel Methodological Setup.

Introduction: Neurophysiologic monitoring can improve autonomic nerve sparing during critical phases of rectal cancer surgery. Objectives: To develop a system for extracorporeal stimulation of sacral nerve roots. Methods: Dedicated software controlled a ten-electrode stimulation array by switching between different electrode configurations and current levels. A built-in impedance and current level measurement assessed the effectiveness of current injection. Intra-anal surface electromyography (sEMG) informed on targeting the sacral nerve roots. Background The estimated incidence of osteoporotic pelvic fractures among persons over age 60 in Germany is 224 per 100 000 persons per year, and rising. A number of surgical treatment options are available, but clinical long-term data are lacking. Methods This review is based on pertinent publications and guidelines retrieved by a selective literature search, and on the authors' clinical experience. Bias artifact corrupts magnetic resonance images in such a way that the image is afflicted by illumination variations. Some of the authors proposed the Exponential Entropy Driven - Homomorphic Unsharp Masking (E2D-HUM) algorithm that corrects this artifact without any a priori hypothesis about the tissues or the Magnetic Resonance image modality. Moreover, E2D-HUM does not care about the body part under examination and does not require any particular training task. Laparoscopic lymph node dissection should be performed before fertility preserving treatment of patients with cervical cancer

Objective: The aim of this study is to assess our results of treatment of women with stage I cervical cancer > 2 cm in diameter seeking fertility preservation. Treatment consisted of Laparoscopic Pelvic and Paraaortic Lymphadenectomy (LPPLND), and when no nodal metastasis was detected, neoadjuvant chemotherapy (NACT) followed by radical vaginal trachelectomy (RVT). Patients with positive lymph nodes underwent primary chemoradiation.
