Noninvasive respiratory support in the hypoxaemic peri-operative/periprocedural patient: A joint ESA/ESICM guideline.

2020

Hypoxaemia is a potential life-threatening yet common complication in the peri-operative and periprocedural patient (e.g. during an invasive procedure at risk of deterioration of gas exchange, such as bronchoscopy). The European Society of Anaesthesiology (ESA) and the European Society of Intensive Care Medicine (ESICM) developed guidelines for the use of noninvasive respiratory support techniques in the hypoxaemic patient in the peri-operative and periprocedural period. The panel outlined five clinical questions regarding treatment with noninvasive respiratory support techniques [conventional oxygen therapy (COT), high flow nasal cannula, noninvasive positive pressure ventilation (NIPPV) a…

Noninvasive Ventilation in the Perioperative Period

2019

High-flow nasal therapy (HFNT), as well as noninvasive ventilation (NIV), represents both noninvasive supports. HFNT is a ventilator support where humidified and heated gases are administered to the patient’s airways by a small nasal cannula at flow that reach up to 60 L/min. The delivered flow can be combined with oxygen providing different inspiratory oxygen fractions (FiO2). HFNT is increasingly used for critically ill adult patients in different clinical settings. Differently from noninvasive ventilation (NIV) which is intended to deliver intermittent positive pressure ventilation (NIPPV) or continuous positive airway pressure (CPAP) to the patient’s airway via different external interf…

General Aspects of Thoracic Anesthesia

2020

Thoracic anesthesia, a fascinating subspecialty, provides perioperative medicine for a spectrum of dynamically evolving surgical procedures ranging from classical thoracotomies to minimally invasive and ultra-minimally invasive video or robotically assisted procedures. Its core mission is to provide optimal surgical conditions by deflating the operative lung while providing adequate oxygenation and avoiding injury to the ventilated lung. Thoracic anesthesia also provides preoperative care to achieve best possible health status before surgery and improves postoperative recovery by providing means of pain therapy and enhanced recovery following some of the most painful procedures in patients …

A qualitative study of perioperative nursing students' experiences of interprofessional simulation-based learning.

2020

Aim: To explore perioperative nursing students' experiences of interprofessional simulation-based learning to gain a deeper understanding of how this educational tool can be used to support students' learning and enable them to achieve the intended learning outcomes. Background: Despite extensive research, it remains unclear what and how participants learn from interprofessional simulation-based learning. There is a need to specify how interprofessional simulation-based learning should be organised to support and promote learning processes, especially for postgraduate learners. In particular, there seems to be little evidence in the existing literature in the field of educating perioperativ…

Problem solving strategies with biomedical and clinimetric components: The case of perioperative H1-+H2-prophylaxis

1992

A formal (objective) problem solving strategy was developed for the question whether a perioperative histamine H1-+H2-prophylaxis should be applied to patients at risk for a histamine release reaction or for a special severity of such a reaction even in the case of a minor histamine release.

2020

Exposed Subcutaneous Implantable Devices

2015

Background: Implantable venous and electrical devices are prone to exposure and infection. Indications for management are controversial, but—especially if infected—exposed devices are often removed and an additional operation is needed to replace the device, causing a delay in chemotherapy and prolonging healing time. We present our protocol for device salvage, on which limited literature is available. Methods: Between 2007 and 2013, 17 patients were treated (12 venous access ports, 3 cardiac pacemakers, and 2 subcutaneous neural stimulators). Most patients were operated within 7 days from exposure. All patients received only a single perioperative dose of prophylactic antibiotic. In cases …

Manejo perioperatorio de los anticoagulantes orales directos en cirugía urgente y sangrado. Monitorización y tratamiento hemostático

2015

There is an almost unanimous consensus on the management of the direct new oral anticoagulants, dabigatran, rivaroxaban, and apixaban in elective surgery. However, this general consensus does not exist in relation with the direct new oral anticoagulants use in emergency surgery, especially in the bleeding patient. For this reason, a literature review was performed using the MEDLINE-PubMed. An analysis was made of the journal articles, reviews, systematic reviews, and practices guidelines published between 2000 and 2014 using the terms "monitoring" and "reversal". From this review, it was shown that the routine tests of blood coagulation, such as the prothrombin time and activated partial th…

Surgical treatment of pulmonary artery sarcoma

2001

Abstract Objective: Pulmonary artery sarcomas are rare and usually fatal tumors. The diagnosis is difficult and delayed in most cases. Newer imaging techniques could allow early diagnosis in patients with symptoms of pulmonary vascular obstruction. Surgical resection improves clinical symptoms and offers the only chance of cure. We report the case histories of 7 patients with primary pulmonary artery sarcomas treated by surgical resection with or without adjuvant therapy. Methods: Seven patients (3 women and 4 men; mean age, 52.3 years; preoperative New York Heart Association functional class III/IV, n=5/2) underwent operations. Malignancy was preoperatively suspected in 5 patients, and 2 p…
