Quantitative and qualitative effects of rosuvastatin on LDL-cholesterol: what is the clinical significance ?

2009

Summary Background:  Statins have emerged as the global leader in pharmacologic therapy for dyslipidaemia, and rosuvastatin has demonstrated clinical efficacy as well as safety in several clinical trials and postmarketing analyses. Aim:  The present article reviewed the effects of rosuvastatin on the quantity and the quality of low-density lipoproteins (LDL). Methods:  We searched for and reviewed all the available evidence in a systematic way. A literature search (by Medline and Scopus) was performed using the following headings: ‘LDL-cholesterol’, ‘LDL size’, ‘LDL subclasses’, ‘small dense LDL’, ‘apolipoprotein B, apo B’ and ‘rosuvastatin’ up to 11 November 2008. The authors also manually…

Medicinal plants in the prevention and treatment of chronic diseases 2013.

2014

Since the dawn of human civilization, human beings have found remedies within their habitat and have adopted different therapeutic strategies depending upon climatic, phytogeographic, sociocultural, floral, and faunal characteristics. Traditional systems thus contain beliefs and practices in order to avoid, prevent, or avert ailments, which constitute traditional preventive medicine. The use of medicinal herbs and herbal medicine is an age-old tradition and the recent progress in modern therapeutics has stimulated the use of natural product worldwide for diverse ailments and diseases. The educated public and health care professionals have enormous interests in the medicinal uses of herbs, b…

Day and Night Control of COPD and Role of Pharmacotherapy: A Review

2020

The topic of 24-hour management of COPD is related to day-to-night symptoms management, specific follow-up and patients' adherence to therapy. COPD symptoms strongly vary during day and night, being worse in the night and early morning. This variability is not always adequately considered in the trials. Night-time symptoms are predictive of higher mortality and more frequent exacerbations; therefore, they should be a target of therapy. During night-time, in COPD patients the supine position is responsible for a different thoracic physiology; moreover, during some sleep phases the vagal stimulation determines increased bronchial secretions, increased blood flow in the bronchial circulation (…

Pharmacotherapy of COPD in Central and Eastern Europe - The POPE study

2016

Introduction: In Central and Eastern Europe (CEE) the burden of chronic obstructive pulmonary disease (COPD) is high. It remains unknown whether patients are treated in line with GOLD recommendations or according to their phenotypes. Our aim was to analyze the treatment patterns of COPD in CEE. Methods: In the frame of the POPE study (ClinicalTrials.gov - NCT02119494) data including pharmacotherapy were collected in patients with stable COPD across 11 CEE countries. Results: 3366 patients (66±9 yrs, 70% male, FEV 1 53±18%) participated in the study. In all GOLD categories, the most widely used medications were long-acting beta-2 agonists (LABA), followed by long-acting antimuscarinic agents…

Optimizing the treatment of chronic hepatitis due to hepatitis C virus genotypes 2 and 3: a review

2009

Recently several randomized trials involving exclusively HCV 2 and 3 patients have explored the possibility of reducing the duration of therapy with PEG IFNs and ribavirin to 12–16 weeks. Among these, the largest studies (ACCELERATE, NORTH-C and NORDynamIC) have failed to demonstrate, by intention-to-treat analysis, that short treatment is non-inferior to the standard duration of 24 weeks originated by phase 3 trials. Even though obtaining univocal conclusions from these studies are difficult to obtain due to some critical differences (trial design, genotypes 2/3 ratio, rate of cirrhosis at baseline, ribavirin dose, assays to detect HCV-RNA etc), all have proved that a rapid virological res…

Clinical Trial Results of Peginterferons in Combination with Ribavirin

2003

Of the large number of patients chronically infected with hepatitis C virus (HCV), only about one third have progressive liver disease, and will eventually develop cirrhosis and hepatocellular carcinoma. These are the patients for whom effective antiviral treatment is most needed. Therapy is currently recommended for patients with chronic hepatitis C who have abnormal alanine aminotransferase (ALT) levels, detectable hepatitis C virus ribonucleic acid (HCV RNA) in the blood, and significant necroinflammatory changes and/or fibrosis on liver biopsy. The current gold standard in terms of treatment efficacy is the combination of peginterferon (PEG-IFN) and ribavirin. The overall sustained viro…

International prescribing practices in obsessive-compulsive disorder (OCD)

2016

Objectives To assess rates of psychotropic medication use in patients with obsessive–compulsive disorder (OCD) in seven different countries on five continents and to compare these with international treatment guidelines. Methods Researchers in the field of OCD were invited to contribute summary statistics on the characteristics of their patients with OCD and on their incidence of psychotropic use. Consistency of summary statistics across countries was evaluated. Results The data came from Brazil (n = 955), Italy (n = 750), South Africa (n = 555), Japan (n = 382), Australia (n = 213), India (n = 202) and Spain (n = 82). The majority (77.9%; n = 2445) of the total sample of 3139 participants …

Clinical management of drug-drug interactions in HCV therapy: Challenges and solutions.

2013

Contains fulltext : 118153.pdf (Publisher’s version ) (Open Access) Hepatitis C virus (HCV) infected patients often take multiple co-medications to treat adverse events related to HCV therapy, or to manage other co-morbidities. Drug-drug interactions associated with this polypharmacy are relatively new to the field of HCV pharmacotherapy. With the advent of the direct-acting antivirals telaprevir and boceprevir, which are both substrates and inhibitors of the cytochrome P450 (CYP) 3A iso-enzyme, knowledge and awareness of drug-drug interactions have become a cornerstone in the evaluation of patients starting and continuing HCV combination therapy. In our opinion, an overview of conducted dr…

Efficacy of combination therapy with angiotensin-converting enzyme inhibitor and calcium channel blocker in hypertension.

2012

There are few clinical trials that provide evidence to support the hypothesis that combined therapies offer a favorable risk-benefit ratio in the reduction of cardiovascular mortality and morbidity. Combined therapies containing an angiotensin-converting enzyme inhibitor (ACEI) with a calcium channel blocker (CCB) is one of the recommended combinations in the reappraisal of the European Society of Hypertension. The authors have performed a systematic review of the available clinical evidence on the use of combined therapies containing an ACEI with a CCB versus other combinations in the management of arterial hypertension (HT) and in the reduction of cardiovascular morbidity/mortality, accord…
