Työtä, sokkeja, lääkkeitä : Siilinjärven piirimielisairaala ja potilaiden hoitomuodot vuosina 1926-1959

Mielisairaiden eristäminen laitoksiin alkoi Suomessa 1600-luvulla. Hospitaalien ja lääninlasarettien kautta syntyi pohja valtion omistamille keskuslaitoksille sekä kuntien ylläpitämille kunnalliskotien mielisairasosastoille ja 1900-luvulla syntyneille piirimielisairaaloille. Aina vuoden 1952 mielisairaslakiin saakka valtion tehtävänä oli hoitaa akuutteja potilaita. Piirimielisairaaloihin puolestaan sijoitettiin kroonikkopotilaat. Tutkielman kohteena on Siilinjärven piirimielisairaala (Harjamäki) ja sen potilaisiin vuosina 1926-1959 käytetyt hoitomuodot. This research explored the conceptualisation of self-injurious behavior (SIB) in terms of similarities and differences amongst the two types of this behavior. It further explored the efficacy of a brief psychological intervention for SIB as well as the effect of a brief training in the assessment and treatment of SIB on the attitudes psychiatric personnel hold towards people who engage in SIB. Background: In the Need-Adapted approach decisions are made in therapy meetings with all relevant parties; this poses a professional challenge to psychiatrists. Aims: To describe psychiatrists’ inner dialogues concerning their workmates in treatment meetings. Methods: Using videos of co-research interviews, stimulated-recall interviews with eight psychiatrists were conducted and transcribed verbatim. The material was analyzed using an adapted form of dialogical analysis, focusing on voices and positioning. This research focused on factors associated with schizophrenia as well as on the feasibility and effectiveness of group metacognitive training (MCT) in schizophrenia patients with a history of violence in a high-security forensic hospital setting. One of the aims was to scrutinize reasoning ability in regards to jumping to conclusions (JTC) cognitive bias and the expression of cognitive insight (insight into one’s own thinking), a metacognitive ability. This dissertation examines the impact that the materiality of institutions has on patients and staff. Fieldwork has been carried out at five district psychiatric centres (Distriktspsykiatriske sentra, DPS) that were originally built as tuberculosis institutions. The patients’ and employees’ experiences of being present in the physical and social space of these institutions has been examined, interpreted and reflected upon. The paper provides knowledge about the significance institutions’ materiality and inner life has on the informants, and how the informants influence the materiality.
