Practice-based learning and innovation in nursing homes

2020

Purpose This paper aims to investigate the conditions under which learning and innovation occur within nursing homes by focusing on how the dynamics of the distribution and transformation of ideas and knowledge may be viewed as a prerequisite for innovation in both formal, planned learning situations and informal, everyday practices. Design/methodology/approach Data was produced via fieldwork, which included participant observations, conversations and research interviews with staff and managers at a Norwegian nursing home. The paper is inspired by situated learning theories and communities of practice, as the social context emerges as the site where learning and innovation are cultivated. …

Isolated superior oblique palsies with electrophysiologically documented brainstem lesions

2000

Over a 13.5-year period, we observed 10 patients with isolated superior oblique palsies in whom electrophysiological abnormalities indicated brainstem lesions. In 7 patients unilateral masseter reflex abnormalities were seen, and were located on the side of the superior oblique palsy in 2 patients and on the opposite side in 5 patients. Two patients had slowed gain of following eye movements to the side contralateral to the superior oblique palsy. Slowed adduction saccades in the eye contralateral to the superior oblique palsy were seen in 1 patient. Clinical improvement was frequently (in 7 of 10 patients) associated with improvement or normalization of electrophysiologic findings. Magneti…

17. Masseter reflex abnormalities with meso-diencephalic lesions

2012

different papers and some eponyms used to characterize a certain neurological state differ from the original description. Except for Wallenberg‘s syndrome, classical brainstem syndromes are rarely seen in clinical practice. Lacunar brainstem syndromes are the consequence of infarcts, which involve long tracts and spare intra-axial cranial nerve segments. This group includes pure motor or pure sensory stroke, dysarthriaclumsy hand syndrome, or ataxic hemiparesis. Such infarcts may also cause body lateropulsion with or without limb ataxia, internuclear ophthalmoplgia, skew-torsion sign, or ocular tilt reaction. Small deep infarcts, which solely affect certain nuclei, may be followed by horizo…

Interaction of somatoform and vestibular disorders

2006

The high coincidence of organic vestibular and somatoform vertigo syndromes has appeared to support pathogenic models showing a strong linkage between them. It was hypothesised that a persisting vestibular dysfunction causes the development of anxiety disorders. To determine the relation between vestibular deficits and somatoform vertigo disorders in an interdisciplinary prospective study.Participants were divided into eight diagnostic groups: healthy volunteers (n=26) and patients with benign paroxysmal positioning vertigo (BPPV, n=11), vestibular neuritis (n=11), Menière's disease (n=7), vestibular migraine (n=15), anxiety (n=23), depression (n=12), or somatoform disorders (n=22). Early-onset ataxia with cardiomyopathy and retained tendon reflexes maps to the friedreich's ataxia locus on chromosome 9q

1995

Absence of lower limb tendon reflexes has been considered an essential diagnostic criterion for Friedreich's ataxia (FA). However, preservation of knee and ankle jerks has been reported in a few patients. Linkage analysis to FA locus (FRDA) on chromosome 9q13-21.1 was performed in 11 patients from 6 families with FA phenotype, including cardiomyopathy, but retained reflexes (FARR). A maximal lod score of 3.38 at recombination fraction theta equal to 0.00 was obtained demonstrating that FARR maps to the FRDA locus. These results suggest that FARR is a variant phenotype of FA.

Prevalence of red ear syndrome in juvenile primary headaches.

2010

Background: Previous studies have suggested a relationship between ‘red ear syndrome’ (RES) and pediatric migraine. Aims of this study were (i) to assess the frequency, specificity and sensitivity of RES in a population of pediatric migraineurs and (ii) to establish the pathophysiological mechanisms of RES associated with migraine. Methods and results: A total of 226 children suffering from headache (aged 4–17 years) were enrolled. One hundred and seventy-two (76.4%) were affected by migraine, the remaining 54 (23.6%) by other primary headaches. RES was followed significantly more frequently by migraine (23.3%; p < .0001), and was characterized by high specificity and positive predictiv…
