A human factors intervention in a hospital - evaluating the outcome of a TeamSTEPPS program in a surgical ward

2021

AbstractBackgroundPatient safety in hospitals is being jeopardized, since too many patients experience adverse events. Most of these adverse events arise from human factors, such as inefficient teamwork and communication failures, and the incidence of adverse events is greatest in the surgical area. Previous research has shown the effect of team training on patient safety culture and on different areas of teamwork. Limited research has investigated teamwork in surgical wards. The aim of this study was to evaluate the professional and organizational outcomes of a team training intervention among healthcare professionals in a surgical ward after 6 and 12 months. Systems Engineering Initiative…

Risk analysis using fuzzy set theory of accidental exposure of medical staff during brachytherapy procedures

2010

Using fuzzy set theory, this paper presents results from risk analyses that explore potential exposure of medical operators working in a high dose rate brachytherapy irradiation plant. In these analyses, the HEART methodology, a first generation method for human reliability analysis, has been employed to evaluate the probability of human error. This technique has been modified on the basis of fuzzy set concepts to take into account, more directly, the uncertainties of the so-called error-promoting factors on which the method is based. Moreover, with regard to some identified accident scenarios, fuzzy potential dose was also evaluated to estimate the relevant risk. The results also provide s…

The Safety Culture Enactment Questionnaire (SCEQ): Theoretical model and empirical validation

2015

This paper presents the Safety Culture Enactment Questionnaire (SCEQ), designed to assess the degree to which safety is an enacted value in the day-to-day running of nuclear power plants (NPPs). The SCEQ is based on a theoretical safety culture model that is manifested in three fundamental components of the functioning and operation of any organization: strategic decisions, human resources practices, and daily activities and behaviors. The extent to which the importance of safety is enacted in each of these three components provides information about the pervasiveness of the safety culture in the NPP. To validate the SCEQ and the model on which it is based, two separate studies were carried…

[Analysis of patient safety culture in an out of hours primary care service.]

2020

Background: Aim of this study, part of a European collaborative research project, was to evaluate the "patient safety culture" (PSC) in a primary care out of hours service in order to provide the management with a baseline for improvement interventions. Methods: Cross sectional study with the administration of the Safety Attitude Questionnaire Ambulatory Version (SAQ-AV) administered to all the 56 doctors working out of hours in the ULSS 20 Verona Local Health Trust in April/May 2015. For each item of the questionnaire the average score, standard deviation, non-applicability, percentage of agreement, percentage of disagreement were calculated. Results: Doctors working out of hours appreciat…

Advanced Signal Processing for Ultrasonic Structural Monitoring of Waveguides, in Bridge Maintenance, Safety Management, Health Monitoring and Inform…

2008

Ultrasonic Guided Waves (UGWs) are a useful tool in those structural health monitoring applications that can benefit from built-in transduction, moderately large inspection ranges and high sensitivity to small flaws. This paper describes a damage detection method, based on wavelet transform and outlier analysis for structural waveguides. The method combines the advantages of UGW inspection with the outcomes of the Discrete Wavelet Transform (DWT) that is used for extracting defect-sensitive features that can be combined to perform a multivariate diagnosis of damage. In particular, the DWT is exploited to generate a set of relevant wavelet coefficients to construct a uni-dimensional or multi…

Are organizations committed to stress management interventions?

2020

Purpose – Work-related stress causes individual, societal and economic costs. Stress management interventions (SMIs) are implemented to decrease those costs, reduce the psychosocial harms and increase workers’ productivity. This article reports on a study of a substantial sample of enterprises to verify what types of SMIs they implemented and how they assessed the results of these interventions. Design/methodology/approach – Specially designed surveys were collected in 2015–2017 from 408 organizations of different sizes and sectors in Poland, including multinationals. The reliability of the questionnaire was satisfactory (Cr. 0.84). Findings – Results show that 59% of enterprises are aware …

Incidence of potential drug interactions in a transplant centre setting and relevance of electronic alerts for clinical practice support

2013

Background Adverse drug events may occur as a result of drug–drug interactions (DDIs). Information technology (IT) systems can be an important decision-making tool for healthcare workers to identify DDIs.Objective The aim of the study is to analyse drug prescriptions in our main hospital units, in order to measure the incidence and severity of potential DDIs. The utility of clinical decision-support systems (CDSSs) and computerised physician order entry (CPOE) in term of alerts adherence was also assessed. DDIs were assessed using a Micromedex healthcare series database. Methods The system, adopted by the hospital, generates alerts for prescriptions with negative interactions and thanks to a…

Improvement of the technological process of carbonated spring water by implementing the quality and food safety management standards

2019

Bottled spring water must be health providing, clean, without microorganisms, parasites or substances. The design, manufacturing and management of processes, related to spring water bottling involves controlling the safety issues that may arise in the production processes. These aspects are not sufficient to prevent hazards and they must be related to the implementation of a quality and food safety systems. The aim of the paper is to improve the technological process of carbonated spring water in Monopolis S.A Company, by implementing the quality and food safety management standards. The methodology is based on the implementation requirements of the HACCP principles, included in ISO 22000 s…

EPR DOSIMETRY IN A MIXED NEUTRON AND GAMMA RADIATION FIELD

2004

Suitability of Electron Paramagnetic Resonance (EPR) spectroscopy for criticality dosimetry was evaluated for tooth enamel, mannose and alanine pellets during the 'international intercomparison of criticality dosimetry techniques' at the SILENE reactor held in Valduc in June 2002, France. These three materials were irradiated in neutron and gamma-ray fields of various relative intensities and spectral distributions in order to evaluate their neutron sensitivity. The neutron response was found to be around 10% for tooth enamel, 45% for mannose and between 40 and 90% for alanine pellets according their type. According to the IAEA recommendations on the early estimate of criticality accident a…
