Pattern of drug use by advanced cancer patients followed at home

The aim of this study was to document the drugs most commonly prescribed to control symptoms in advanced cancer patients being followed at home. We analyzed data for 128 patients admitted to a home palliative care program from January 1993 to January 1995. All patients were followed at home until death by a team consisting of doctors and nurses, and were given two or three medical examinations a week. The most frequently prescribed drugs were analgesics and drugs commonly used to prevent NSAID-induced gastric toxicity. Slow-release morphine was the analgesic used most often. The recent introduction of new drugs such as capecitabine, irinotecan, and oxaliplatinum has greatly improved the clinical outcome of patients with advanced/metastatic colorectal cancer. Nevertheless, some patients may suffer from the adverse drug reactions which will probably be the main cause of chemotherapy failure. Several drugs, shown to be safe for other uses, have proven to be highly effective adjuncts for ovarian stimulation. The authors evaluate these "orphan" indications and make recommendations so that more patients will benefit from their use.

Public service workers in the frontline have traditionally enjoyed a wide freedom to make decisions during policy implementation. Research shows that technology has both constraining and enabling effects on public service workers affecting their ability to exercise discretion. What remains unclear is under which circumstances discretion is influenced by technology. Using a case study approach and drawing on neo-institutional theory, this paper studies a court to identify contextual factors affecting the phenomenon. In our study, we test users’ color preferences of interfaces of typical electronic services’ (e-services) environment. The motivation of our study is to explore discrepancy between (a) the high degree of availability of e-services in Latvia, and (b) low usage of these e-services among population.
