In conjunction with the dismantling of psychiatric hospitals, social workers have been commissioned to help service users in their daily living in their homes and in the community. The consequences of these changes for experience-based knowledge and practices in their contexts remain relatively unknown. In this study, eighteen service users and the social workers they described as helpful for them were interviewed. The interviews were recorded, transcribed, and analyzed using Thematic Analysis. This study examines how disability service workers identify the discourses of the client-worker relationships. We studied the clientworker relationship from the perspective of the relational dialectics theory with a focus on relational contradictions and the meanings created within discursive struggles. Uno de los argumentos más comunes a la hora de justificar el exiguo impacto d elos servicios sociales es el escaso peso que éstos representan en el conjunto del gasto social. En el articulo se muestra que esta justificación puede no ser del todo cierta. Derivation and validation of a novel bleeding risk score for elderly patients with venous thromboembolism on extended anticoagulation

SummaryExisting clinical scores do not perform well in predicting bleeding in elderly patients with acute venous thromboembolism (VTE). We sought to derive an easy-to-use clinical score to help physicians identify elderly patients with VTE who are at high-risk of bleeding during extended anticoagulation (>3 months). Our derivation sample included 743 patients aged ≥65 years with VTE who were enrolled in a prospective multicenter cohort study. BackgroundSurvivors of childhood cancer are at risk of subsequent primary neoplasms (SPNs), but the risk of developing specific digestive SPNs beyond age 40 years remains uncertain. We investigated risks of specific digestive SPNs within the largest available cohort worldwide.MethodsThe PanCareSurFup cohort includes 69 460 five-year survivors of childhood cancer from 12 countries in Europe.
