Endothelin-1 concentrations in the internal jugular and azygous veins in multiple sclerosis patients: the results of a pilot study
2014
Objectives: In this pilot study we examined the potent vasoconstrictor, endothelin-1, in the blood of multiple sclerosis patients in the context of chronic cerebrospinal venous insufficiency hypothesis. For this purpose we measured endothelin-1 concentrations in blood samples that were obtained during selective catheterisation of the main veins draining the central nervous system: the internal jugular veins and the azygous vein. Material and methods: We measured endothelin-1 concentrations in peripheral blood in nine multiple sclerosis patients and five healthy controls. In multiple sclerosis patients this peptide was also evaluated in blood samples obtained from the internal jugular veins …
Effects of wrist oscillations on contralateral neurological postural tremor using a new myohaptic device ('wristalyzer')
2007
Upper limb postural tremor consists of distinct oscillations, mainly mechanical-reflex and central-neurogenic. These oscillations are superimposed upon a background of irregular fluctuations in muscle force. Muscle spindles play key-roles in the information flow to supra-spinal and spinal generators. Oscillations were delivered using a new generation portable myohaptic device, called 'wristalyzer', which takes into account the ergonomy of upper limbs allowing a fine adjustment to each configuration of upper limb segments. The nominal torque of the device is 4 Nm, with a maximal rotation velocity of 300 degrees/s and a range of motion of +45 to -45 degrees. Reliability was assessed in basal …
The Stretch-Shortening Cycle
2006
Neuromuscular fatigue has traditionally been examined using isolated forms of either isometric, concentric or eccentric actions. However, none of these actions are naturally occurring in human (or animal) ground locomotion. The basic muscle function is defined as the stretch-shortening cycle (SSC), where the preactivated muscle is first stretched (eccentric action) and then followed by the shortening (concentric) action. As the SSC taxes the skeletal muscles very strongly mechanically, its influence on the reflex activation becomes apparent and very different from the isolated forms of muscle actions mentioned above. The ground contact phases of running, jumping and hopping etc. are example…

Intraoperative Sonographie bei lumbalen Bandscheibenvorfällen
2008
UNLABELLED Intraoperative ultrasound of the spine has been used in neurosurgery (tumours) and traumatology (reposition of fractured vertebral bodies fragments). AIM To describe initial experiences with this method and to document the opportunities and problems. METHOD 24 nucleotomies were documented and evaluated. Sonography was performed using a 7.5 MHz sector probe and a typical surgical approach. RESULTS In 21 of 24 cases, imaging of intraspinal structures was possible. The complete extraction of the herniated disk could be documented. In 3 cases the examination failed because the probe was too large to be pushed down between the spinous processes and the wound retractor to the operative…
Diagnosis, treatment, and analysis of long-term outcomes in idiopathic normal-pressure hydrocephalus
2007
To the Editor:We read with interest the article by McGirt et al. (9) entitled“Diagnosis, Treatment, and Analysis of Long-term Outcomes inIdiopathic Normal Pressure Hydrocephalus.” The authors con-clude that gait impairment is the primary symptom that inde-pendently predicted improvement after shunting. The studyincluded 132 patients, 129 (98%) of whom had gait impairmentas a feature. We would like to comment on this particular selec-tion criterion, as well as improvements in the Mini Mental StateExamination (MMSE) as the sole measure of postoperative cog-nitive improvement. One of the inclusion criteria for this study was presentationwith two or more features of the classic triad. Thus, the …
Chronic cerebrospinal venous insufficiency: current perspectives
2014
This review summarizes the research to date on chronic cerebrospinal venous insufficiency (CCSVI). CCSVI was initially defined as a clinical syndrome comprising stenoses of the internal jugular and/or azygos veins, characterized by collateral venous outflows and reduced cerebral blood flow, and was found primarily in patients with multiple sclerosis. However, the published evidence on CCSVI is very discordant. Catheter venography studies gave a regular picture, with the majority of patients with multiple sclerosis presenting with demonstrable outflow abnormalities in the veins draining the central nervous system. The prevalence of these lesions was over 50%, and even higher (about 90%) when…
