The automated pancreas: A review of technologies and clinical practice

2021

Insulin pumps and glucose sensors are effective in improving diabetes therapy and reducing acute complications. The combination of both devices using an algorithm-driven interoperable controller makes automated insulin delivery (AID) systems possible. Many AID systems have been tested in clinical trials and have proven safety and effectiveness. However, currently, none of these systems are available for routine use in children younger than 6 years in Europe. For continued use, both users and prescribers must have sound knowledge of the features of the individual AID systems. Presently, all systems require various user interactions (e.g. meal announcements) because fully automated systems ar…

Treatment Compliance in the Long-Term Paranoid Schizophrenia Telemedicine Study

2017

Schizophrenia is a chronic disease with a relatively high relapse rate. Different methods are introduced to improve compliance of the patients treated by psychiatrists; among them, a new and promising attitude is telepsychiatry. The 12-month clinical study of the compliance in paranoid schizophrenia was performed on the group of 199 patients. Every patient in the study was given a smartphone with the preinstalled original telemedicine platform (Telemedicine MoneoPlatform). The telemedicine system recorded every confirmation of the drug intake, and according to that, the compliance was counted as the percent of the doses of medication confirmed in relation to the planned ones. In the first m…

Pregnancy e-health: a multicenter Italian cross-sectional study on internet use and decision-making among pregnant women

2013

Background Our study aimed to estimate the prevalence of pregnancy e-health seekers in a large Italian sample; to explore the factors influencing the choices of the childbearing women regarding their lifestyles after internet consultation; and finally to investigate potential differences between primiparous and multiparous women in internet use to find information about pregnancy. Methods A multicentre survey was carried out in seven Italian cities. Data were collected through a validated questionnaire administered in waiting rooms of outpatient departments by medical doctors. Respondents were questioned about their sociodemographic status, their use of the internet to seek pregnancy inform…

Face-to-Face with the Doctor Online: Phenomenological Analysis of Patient Experience of Teleconsultation.

2022

The global crisis of the COVID-19 pandemic has considerably accelerated the adoption of teleconsultation—a form of consultation between patient and health care professional that occurs via videoconferencing platforms. For this reason, it is important to investigate the way in which this form of interaction modifies the nature of the clinical encounter and the extent to which this modification impacts the healing process. For this purpose, I will refer to insights into the clinical encounter as a face-to-face encounter drawn from the phenomenology of medicine (R. Zaner, K. Toombs, E. Pellegrino). I will also take into account a criticism that has been expressed by various contemporary phenom…

Background: Excessive gestational weight gain is common and associated with adverse outcomes both in the short and long term. Although traditional lifestyle-based interventions have shown to mitigate excess gestational weight gain, little is known about whether mobile Health (mHealth) apps can promote healthy weight gain, diet, and physical activity during pregnancy. Objective: The primary aim of the HealthyMoms trial is to determine the effectiveness of a smartphone app (HealthyMoms) for mitigating excess gestational weight gain during pregnancy. Secondary aims are to determine the effectiveness of the app on dietary habits, physical activity, body fatness, and glycemia during pregnancy. M…

Friction forces and patient-centredness: Understanding how established logics endure during infrastructure transformation

2017

n this article, we examine three cases of e-health solutions for patients in Norway. For the analysis of the three cases, we focused on friction forces that come into play when different established arrangements need to change to accommodate novelty. We argue that the design of new technologies was shaped by friction related to institutionalised practices, regulatory regimes and entrenched patient roles. These friction forces connect the past with the present, come into action when aiming for novelty and result to the perpetuation of constituents of the past during change processes. Specifically, the e-health solutions under study were strongly influenced by established healthcare provision…

Implementation plan for stroke telemedicine in the Valencian Region

2016

Stroke telemedicine is a counselling modality that enables care of stroke patients at remote departmental hospitals by specialists at stroke units in reference hospitals. The planning and implementation of a hub-and-spoke telestroke network are complicated. This paper describes the implementation plan of Telestroke in the Valencian Region, Spain. A stroke telemedicine network was installed in the Valencian Region connecting the departmental hospitals with stroke teams, to the 6 public reference hospitals with a stroke unit. The implementation plan included the installation of the stroke telemedicine network with availability of connection over the internet by VPN. Strategies to ensure good …

Utilización de tecnologías móviles en pacientes con psicosis: una revisión sistemática.

2017

[EN] There is a growing interest in mobile Health interventions (m-Health) in patients with psychosis. The aim of this study is to conduct a systematic review in order to analysethe current state of research in this area. The search of articles was carried out following the PRISMA criteria, focusing on those studies that used mobile technologies in patients with psychosis during the period from 1990 to 2016. A total of 20 articles were selected from the 431 studies found. Three types of studies are distinguished: 1) Analysis of quality and usability, 2) Improving treatment adherence and reducing hospital admissions, and 3) Analysisof patient symptoms. CONCLUSIONS: m-Health interventions are…

Management of severe asthma during the first lockdown phase of SARS-CoV-2 pandemic: Tips for facing the second wave.

2022

SARS-CoV-2 pandemic has contributed to implement telemedicine, allowing clinicians to follow the patient remotely, therefore minimizing the risk of any exposure to positive COVID-19 patients. We summarize the approaches adopted to treat and monitor severe asthmatic patients during the lockdown phase of the pandemic. Our experience supports the strategy that every effort should be made to minimize patient contact with the health-care system, planning a pathway that allows patients to receive appropriate medical care and continue the biological therapies, thus preventing the loss of disease control and acute severe exacerbations.

Prioritizing care for severe asthma during SARS-CoV-2 pandemic

2021

Background Almost the entire World is experiencing the Coronavirus-Disease-2019 (COVID-19) pandemic, responsible, at the end of May 2020, of more than five million people infected worldwide and about 350,000 deaths. In this context, a deep reorganization of allergy clinics, in order to ensure proper diagnosis and care despite of social distancing measures expose, is needed. Main text The reorganization of allergy clinics should include programmed checks for severe and poorly controlled patients, application of digital medicine service for mild-to-moderate disease in well-controlled ones, postponement of non urgent diagnostic work-ups and domiciliation of therapies, whenever possible. As far…
