The morphological-functional study of microcirculation is of fundamental importance; in fact, the microvascular bed is directly involved both in autoimmune etiopathogenesis pathologies, and in acute and chronic inflammatory etiopathogenesis pathologies. Oral capillaroscopy is a very stimulating method for studying microcirculation, because of the possibility of studying small vessels in vivo by means of a microscope. Today, it is becoming more reliable thanks to the improvement of the observation tools (photography, videomicroscopy).

We report a rare case of a patient who was referred to the Department of Oral medicine in Zagreb, Croatia. The patient was 20 years old, otherwise healthy and not taking any medication. She presented with irregular erosions partially covered with pseudomembranes that involved both lips and retrocomissural mucosa. Discrete erosion was also noticed on her lower lingual gingiva in the area 42. She reported a propolis solution self-medication for treatment of recurrent aphthous ulcers. After ten days of propolis application, lip and oral lesions developed. Patch test to propolis was proven. Objective: To evaluate the efficacy of switching to etanercept treatment in patients with rheumatoid arthritis who already responded to infliximab, but presented side effects. Methods: Charts of 553 patients with rheumatoid arthritis were retrospectively reviewed to select patients who responded to the treatment with infliximab and switched to etanercept because of occurrence of adverse effects. Fino a pochi decenni addietro, il fenomeno dell’esposizione a basse concentrazioni di agenti tossici era considerato soltanto un problema di igiene ambientale. In realtà vari studi1-5 hanno approfondito, grazie al monitoraggio biologico, il concetto di “esposizione a basse dosi” di agenti tossici o elementi in tracce: pertanto nei suddetti studi, gli autori concludono che le “basse dosi” corrispondono a una esposizione tale che le concentrazioni della sostanza in esame si mantengono, nei fluidi biologici (per la popolazione esposta), al di sopra dei valori massimi di riferimento (stabiliti per la popolazione generale) e al di sotto dei valori-limite biologici.
