Looking for the Easy Way Out : Demographic Panic and the Twists and Turns of Long-Term Care Policy in Finland

This chapter aims to map the twists and turns in the development of long-term care policy in Finland since the start of the 1990s. The main argument of the chapter is that these changes have to a large part been motivated by what is called here ‘demographic panic’, that is, fear of consequences of population ageing to the public purse. Various policy changes have taken place during the last three decades. Since the late 1980s, Finnish long-term care policies have been made under the shadow of the ‘demographic time bomb’ discourse that argues that care expenditures are to rise exponentially if determined action is not taken to curb the expenditures. Vanhukset asiakkaina : osallisuuden tulkintoja vanhuspalvelusopimuksesta

Vanhuspalvelulaki tuli voimaan 1.7.2013. Tutkimuksessani kiinnostukseni kohdistui vanhusten käsityksiin ja kokemuksiin osallisuudestaan vanhustenhuollon asiakkaina. Tutkimusaineisto on tehty haastattelemalla vanhusasiakkaita, joiden kanssa oli tehty vanhuspalvelusopimus. Haastateltavia oli kaikenkaikkiaan kuusi, mutta käytännössä viisi, koska pariskunnasta mies jätti vastaamisen vaimolleen. Tutkimuksessani korostui myös haastateltavien asuinympäristönä maaseutu, josta tällä hetkellä palvelut vähenevät ja nuoriso muuttaa kasvukeskuksiin. Artikkelissa analysoidaan asiakasturvallisuuden määrittymistä vanhuspalveluita koskevissa politiikka-asiakirjoissa. Asiakasturvallisuutta lähestytään kysymyksenä yhteiskunnallisesta sekä asiakassuhteissa toteutuvasta luottamuksesta. Artikkelissa kysytään, miten asiakasturvallisuus määrittyy asiakirjoissa ja mitä se tarkoittaa luottamuksen näkökulmasta. Tulosten mukaan asiakasturvallisuuden sisältö ja merkitys suhteessa potilasturvallisuuden käsitteeseen on vanhuspalveluissa vasta jäsentymässä. AbstractConsumer choice models have been introduced in eldercare services in several Western welfare societies. Choice models in eldercare emphasise the importance of individuals’ abilities to make informed choices and therefore entail a risk for increased inequalities among older adults with care needs. Older peoples' independent living in their own homes and their ability to move around and maintain social relations in their communities have been acknowledged as important aims to support their well-being. In this article, we study Special Transport Services as a means to support older persons with ‘aging in place’ – that is, within their communities. From “go-alongs” (shared dial-a-ride bus journeys) and “sit-down interviews” (at participant's home or a café) with 12 older people living in a Finnish suburb, we explored their everyday dial-a-ride bus trips to the local mall. Care poverty : unmet care needs in a Nordic welfare state

This article introduces the concept of care poverty, defined as inadequate coverage of care needs resulting from an interplay between individual and societal factors, and examines its level and predictors among the 75+ population in Finland. The data come from a survey conducted in 2010 and 2015. Despite the universalistic goals of the Finnish care system, 26 percent of respondents with limitations in daily activities faced care poverty with regard to instrumental activities of daily living; the activities of daily living care poverty rate was 17 percent. Outsourcing within the Nordic context: Care services for older people in Finland.

The early 1990s economic setback brought significant reforms favoring the outsourcing of care in Finnish municipalities. Here, outsourcing refers to the practice of municipalities employing private organizations through different means (e.g., open tendering) to deliver public care services. In this context, this study examines the growth in the outsourcing of service housing and home-help services in 311 municipalities from 2001 to 2015 and investigates the municipal factors associated with outsourcing using four dimensions: care needs, population size, economic situation, and political ideology of the municipality.
