Deliberate Arterial Hypotension Does Not Reduce Intraocular Pressure in Pigs
Among the accepted advantages of general anesthesia in ophthalmic surgery is improved control of intraocular pressure (IOP). Although standard textbooks advocate deliberate arterial hypotension to facilitate intraocular surgery by reducing IOP, scientific proof of such an effect is lacking. The authors investigated effects of induced arterial hypotension on IOP in an anesthetized porcine model. Forty-two piglets were anesthetized with piritramide, were placed in the prone position, and had the anterior chamber of one eye punctured with a small Teflon cannula to measure IOP. Aplasia of the retinal vessels combined with optic nerve hypoplasia, neonatal epileptic seizures, and lactic acidosis due to mitochondrial complex I deficiency.
A newborn male with mitochondrial complex I deficiency suffered from neonatal epileptic seizures, which later developed into infantile spasms. The infant was blind due to aplasia of the retinal vessels and hypoplasia of the optic nerve. There was congenital lactic acidosis, which persisted in later life. The boy was microcephalic and retarded. Muscular hypotonia later shifted to spasticity. Succinic acid was increased in urine. We assume that the aplasia of the retinal vessels is due to damage of the retinal ganglion cells caused by the mitochondrial disease in the first 3 to 4 months of pregnancy.
Echographic patterns of an orbital myxoma and schwannoma
A case of intramuscular orbital myxoma and of a schwannoma of the orbit were studied with B-scan and standardized A-scan echography as described by Ossoinig. The myxoma appeared as a well outlined process with medium internal reflectivity and areas of low internal reflectivity. Postoperatively the incompletely excised tumour was followed with echography as well as CT and MRI. The second tumour, a schwannoma had a very sharply defined shape, with a thick low to medium reflective outer wall and low internal reflectivity. This corresponds to the Antoni A and Antoni B patterns of the excised tumour.
