Treatment of Idiopathic Varicoceles by Transfemoral Testicular Vein Occlusion
Percutaneous transfemoral occlusion of the testicular vein is a new alternative in the treatment of idiopathic varicocele. Embolization with the Gianturco coil was done on 27 patients. The occlusion was performed immediately after diagnostic phlebography. The results of followup for 1 year are available for 18 patients. In 1 patient the coil had to be removed because of persistent pain, although cause and effect could not be proved. Two patients had a recurrence. The urographic findings were normal 1 year after embolization. No dislocation of the foreign body was observed. Two patients reported that their wives were pregnant.
Diagnostische Aussagekraft von Sonographie und Computertomographie bei Lebermetastasen
The diagnostic accuracy of sonography and computed tomography for detecting liver metastases was analysed in 526 patients with tumors and the findings were compared with those at laparotomy and at post mortem. The surgical protocols were analysed in respect of the primary tumour, the frequency of metastases and the number, localisation and size of metastases. A comparison of sonography and CT showed a higher sensitivity for CT (57.1%) compared with sonography (52.3%) although specificity was slightly higher for sonography. Liver metastases greater than 15 mm were detected by sonography (80.5%) and CT (92%), with much higher sensitivity. CT also proved superior, since the results depended le…
Preoperative Radiochemotherapy and Radical Surgery of Advanced Head and Neck Cancers — Results of a Prospective, Multicenter DÖSAK Study
Present research concerning the efficiacy of therapeutic strategies for squamous cell carcinomas of the oral cavity and the oropharynx profits from former observational studies. The so-called retrospective German, Austrian, and Swiss Association for Head and Neck Tumors (DOSAK) study (Platz et al. 1986) served to review existing proposals for classification (UICC 1973, 1978; AJC 1978) and to develop a statistical classification which enables the head and neck oncologist to define prognostically homogeneous patient collectives. The treatment-dependent prognostic index (TPI) was the corresponding result (Platz et al. 1982).
Die Diagnostik des hilären Cholangiokarzinoms: Wertigkeit von präoperativer ERC, MRC und PTC im Vergleich zur Histopathologie
PURPOSE To compare the results of the preoperative workup consisting of endoscopic retrograde cholangiography (ERC), magnetic resonance cholangiography (MRC), and percutaneous resonance cholangiography (PTC) with the tumor extent of the surgical specimen in patients with hilar cholangiocarcinoma (hilCC). MATERIALS AND METHODS Between 9/1997 and 12/2002, 59 patients with hilCC tumor underwent surgical resection. Preoperative ERC, MRC, and PTC were analyzed, blinded for the identity of the patient, and compared with the surgical specimen. For this retrospective analysis, 55 of the initial 59 ERCs, 39 of the initial 40 MRCs and 32 of the initial 38 PTCs were available. Most of the ERCs and MRC…
Follow-up results of embolization therapy for palliation of incurable bladder carcinoma
Embolization therapy of the renal artery (N=10), hypogastric arteries (N=18) and ureter (N=24) was performed in 41 patients with incurable bladder carcinoma for palliation of gross hematuria and severe dysuria. Careful follow-up of the patients until death gave a mean survival time of 12.3 months after embolization; 6 patients are still alive. The results show that embolization therapy is justified for management of bleeding and dysuric complaints. Ureteral occlusion alone has almost no effect on hematuria. Combined embolization procedures yield better results than occlusion of the vessels or the ureter. Ureteral occlusion with a detachable balloon is superior to embolization with tissue ad…
