The influence of penbutolol and placebo on blood sugar levels and insulin consumption in the glucose-controlled insulin infusion system ("artificial endocrine pancreas").
The aim of the study was to investigate the influence of 40 mg of the beta-blocker penbutolol (Betapressin TM; Hoechst Ltd., Frankfurt/Main) in comparison to placebo on the insulin consumption on the blood sugar profile in twelve insulin-dependent diabetes (IDDM) patients. The patients were treated with penbutolol and placebo for a period of three days, and then were examined with the help of the glucose-controlled insulin infusion system. The blood sugar profile and insulin consumption over a 24 hour period was not affected by either penbutolol or placebo, nor could any changes be measured in these parameters when measured after food intake. Therefore, we developed a computerized system which adapts insulin doses using a control matrix. During a first in-patient period we had a remarkable blood glucose normalization and stabilization.
The influence of testosterone substitution on bone mineral density in patients with Klinefelter's syndrome.
The aim of this study was to clarify the extent of bone mineral deficiency in patients with Klinefelter's syndrome on the premise that testosterone substitution could prevent this deficiency. Bone mineral density was measured by single-photon absorptiometry in 42 patients with Klinefelter's syndrome, (21 patients without therapy, 10 with testosterone substitution before the age of 20 and 11 patients with testosterone substitution beginning after the age of 20). We found significantly lower bone mineral density in patients without therapy and in patients when the therapy began later compared to normal individuals. Hematopoietic responses in patients with advanced malignancy treated with recombinant human granulocyte-macrophage colony-stimulating factor.
The in vivo effect of yeast-derived recombinant human granulocyte-macrophage colony-stimulating factor (rh GM-CSF) was investigated in 30 patients with advanced malignancy in a phase Ib trial. Patients were treated at four different dose levels (120 to 1,000 micrograms/m2/d) by either daily intravenous (IV) bolus injection or 24-hour continuous infusion. Administration of rh GM-CSF resulted in a broad spectrum of dose- and schedule-dependent hematopoietic effects. The influence of different generations of computer algorithms on diabetes control
With all control schedules, the management of diabetes is possible using Skyler's algorithm. In general, those control algorithms which do not allow the individual adaptation to changing conditions lead to overinsulinisation. So-called meal-related algorithms do usually minimise the fluctuations in blood sugar. The introduction of self-adapting algorithms, detecting peripheral insulin resistance, may further improve metabolic diabetes control.

Treatment of patients having myelodysplastic a syndromes (MDS) with approaches such as differentiation induction, single cytostatic agents or supportive care only has, up to now, been rather unsuccessful. Aggressive chemotherapy followed by bone marrow transplantation is only suitable for a very small proportion of patients. Thus, there is a need for new therapeutic alternatives.
Knochenveränderungen bei Akromegalie
An 2 7 Frauen und 11 Mannern mit Akromegalie wurde die Knochendichte in Single-Photonenabsorptionstechnik am proximalen (uberwiegend Kompakta) und distalen (uberwiegend Spongiosa) Mespunkt des Unterarms untersucht. Die Knochendichte lag dabei am proximalen Mespunkt im zu erwartenden Altersnormbereich, am distalen Mespunkt war sie deutlich erhoht (p < 0,001). Die mit quantitativer Computertomographie gemessene Spongiosa der Lendenwirbelsaule fanden wir uber das Alter hinaus vermindert. Als Ausdruck eines gesteigerten Knochenabbaus fanden wir das Hydroxyprolin im Urin erhoht, wahrend sich das Osteocalcin als Marker der Osteoblastenaktivitat invers dazu verhielt.

Theoretischer Hintergrund: Der Fragebogen zur Krankheitsverarbeitung (FKV-LIS; Muthny, 1989 ) ist ein im deutschsprachigem Raum häufig eingesetzter Fragebogen zur Messung von Krankheitsbewältigung. Fragestellung: Das Ziel der vorliegenden Studie ist die Überprüfung der Subskalen-Reliabilität und -Spezifität bei unterschiedlichen Erkrankungsgruppen. Methode: Die Gütekriterien der FKV-LIS Skalen werden bei Patienten mit chronisch-entzündlichen Darmerkrankungen (n = 1265), Typ-I-Diabetes (n = 552) oder Harnblasenkarzinom (n = 81) berechnet.
