Sonographie bei Skelettmetastasen - Stellenwert im Vergleich zur Computertomographie und konventionellen Röntgendiagnostik

The value of CT, conventional radiography and ultrasound was compared in 40 patients with 52 skeletal metastases. For superficial lesions, the combination of sonography and conventional radiography was equivalent to CT in showing the extent of bone destruction and soft tissue involvement. As expected sonography was unsatisfactory for deeper lesions. Sonography can provide useful information for planning radiation therapy for superficial lesions, but it does not replace conventional radiography.

Abstract A multicentric, randomized study of squamous cell carcinoma (SCC) of the oral cavity and the oropharynx has been undertaken by DOSAK. The results after radical surgery alone have been compared with the results of combined preoperative radiochemotherapy followed by radical surgery. Patients with primary (biopsy proven) SCC of the oral cavity or the oropharynx with tumor nodes metastasis (TNM) stages T2–4, N0–3, M0 were included in the study. A total of 141 patients were treated by radical surgery alone, whereas 127 patients were treated by radical surgery preceded by preoperative radiochemotherapy. 222. Die pr�operative Bestrahlung beim Rectumcarcinom ?Erfahrungsbericht �ber eine seit 9 Jahren laufende Studie

251 Patienten mit Rectumcarcinom der Stadien T2-4, N x , M0 wurden prospektiv randomisiert. Eine Gruppe mit 34,5 Gy vorbestrahlt und operiert, die andere Gruppe nur operiert. Operationsverfahren waren tiefe vordere Resektion und abdomino-perineale Rectumexstirpation.Ergebnisse: Es ergab sich kein Unterschied in der postoperativen Letalitat, die Dauer der abdominellen Wundheilung und dem Auftreten von Fernmetastasen. Bei Vorbestrahlung ergab sich eine grosere Resektionsrate, eine Verminderung der Anzahl der perinealen Rezidive und eine Verzogerung der perinealen Wundheilung.

MRI-assisted radiation therapy planning of brain tumors--clinical experiences in 17 patients.

A new and simple method for precise determination of lateral opposed treatment portals was developed and used in 17 patients. Compared to CT, MRI led to significant changes of portals in 59% (10/17) of cases. Individual shielding blocks could be precisely designed by use of our new method. MRI is the procedure of choice in local radiation therapy planning of brain tumors.

210. Das Ewing-Sarkom ? Erfahrungen mit der gliedmassenerhaltenden, chirurgischen, radiologischen und chemotherapeutischen Kombinationstherapie

Seit 1981 wird im Mainzer Universitatsklinikum vor allem bei peripher lokalisierten Ewing-Sarkomen eine Kombinationsbehandlung durchgefuhrt, die ohne erhohtes Uberlebensrisiko eine bleibende Funktionstuchtigkeit der betroffenen Extremitat ermoglicht. Es handelt sich hierbei um eine zeitlich genau aufeinander abgestimmte Anwendung 4 verschiedener Chemotherapeutica, einer lokalen Resektion des tumortragenden Knochenabschnittes und einer von 60 auf 36 Gy reduzierten Rontgennachbestrahlung, die strahlenbedingte Spatschaden weitgehend vermeidet und zur Rezidivprophylaxe ausreicht. Graves ophthalmopathy: role of MR imaging in radiation therapy.

Twenty-three patients with Graves ophthalmopathy who underwent radiation therapy were monitored by means of magnetic resonance (MR) imaging. T2 relaxation times of extraocular muscles and orbital fat, areas of extraocular muscles, and degree of exophthalmos were measured by means of MR imaging at the beginning, at the end, and 3 months after completion of radiation therapy. As a result, patients with primarily elevated T2 times of extraocular muscles showed a better therapy response regarding muscle thickening than patients with primarily normal T2 times. Pelvic side wall recurrences of gynecological malignancies subsequent to primary or adjuvant radiation therapy have a bad prognosis, although in almost half of the cases, distant metastases are not present at the time of diagnosis of the local tumor progression. A radical operative resection of the pelvic wall recurrence is rarely possible and a second conventional percutaneous or intracavitary irradiation with a tumoricidal dose is no longer feasible, because of the limited radiation tolerance of the surrounding pelvic tissue.
