Adaptation and Implementation of an Intervention Programme on Spanish Carers and Adolescent Patients With an Eating Disorder: Study Protocol of a Randomized Controlled Trial
Introduction:One of the major problems with inpatient treatment of adolescent girls with an eating disorder (ED) is that the strategies learned during their hospital stay are not easily applied or maintained in their daily lives, and this has been related to high rates of relapse and readmission. The ECHOMANTRA programme was developed to optimize outcomes during and following inpatient or day-patient treatment. ECHOMANTRA is based on interventions for carers (Experienced Carers Helping Others, ECHO) and patients (Maudsley Model of Anorexia Nervosa Treatment for Adults, MANTRA) and is developed from the cognitive interpersonal model of anorexia (Schmidt and Treasure, 2006;Treasure and Schmid…
Intrusive Thoughts in Obsessive-Compulsive Disorder and Eating Disorder Patients: A Differential Analysis
The present study aims to compare the unwanted intrusions experienced by obsessive–compulsive (OCD) and eating disorder (ED) patients, their appraisals, and their control strategies and analyse which variables predict the intrusions' disruption and emotional disturbance in each group. Seventy-nine OCD and 177 ED patients completed two equivalent self-reports designed to assess OCD-related and ED-related intrusions, their dysfunctional appraisals, and associated control strategies. OCD and ED patients experienced intrusions with comparable frequency and emotional disturbance, but OCD patients experienced greater disruption. Differences appeared between groups on some appraisals and control s…
The Role of Negative Affect in Emotional Processing of Food-Related Images in Eating Disorders and Obesity
The aim of the present study was to analyze differences in the emotional processing (valence, arousal, and dominance) of food-related information in patients with eating disorders (ED), patients with obesity, and healthy women. Moreover, the mediator role of negative affect and the moderating role of the diagnostic group (ED vs. non-ED) were analyzed. Method: The sample consisted of 94 women (39 with eating disorders, 19 with obesity, and 36 healthy participants). Measures: International Affective Picture System (IAPS) food picture exposure task; Self-Assessment Manikin Analog-Visual Scale (SAM) appraising Arousal, Valence, and Dominance; Eating Attitudes Test (EAT-26); Positive and Negativ…
Confirmatory factor analysis and psychometric properties of the Yale–Brown–Cornell Eating Disorders Scale Self-Report version (SR-YBC-EDS) in Spanish clinical and non-clinical samples
[EN] Objective: The aimof the study was to adapt and validate the Yale Brown Cornell Eating Disorder Scale (YBC-EDS) transformed into a self-report format in Spanish clinical and non-clinical samples. Method: Eighty-three eating disordered patients and 358 non-clinical participants completed the Self Report-YBCEDS version (SR-YBC-EDS), the Eating Attitudes Test (EAT), and the Penn State Worry Questionnaire (PSWQ). Results: Confirmatory factor analyses of a two-factor second-order model showed adequate values of goodness-of fit indices for non-clinical (normed ÷2=13.4578; df=18; NFI=0.980; GFI=1.00; RMSEA= 0.00) and clinical samples (normed ÷2 = 26.5913; df =18; NFI = 0.944; GFI = 0.981; RMS…
Efficacy of a mental health app intervention on family members of OCD patients
INTRODUCION. Obsessive-compulsive disorder (OCD) has a high cost for families, who frequently take part in compulsions, reassure patients, assume part of their responsibilities, try to conceal the disorder, and show stigmatizing attitudes (Ociskova et al., 2013; Stengler-Wenzke et al., 2004). esTOCma is a gamified mental health mobile application (app) that offers information about OCD, suggests where to find help, and helps fight stigmatizing attitudes. The objective of this study is to test esTOCma efficacy in a sample of OCD family members. Specifically, we will analyze if, after using the app, there is a change in the following variables: mental health literacy about OCD, stigma and soc…
Mental health literacy and stigma associated with obsessive-compulsive content dimensions: effectiveness of an intervention via smartphone.
Introduction. Obsessive-compulsive disorder (OCD) is a serious and disabling mental illness that presents a high delay in help seeking. Various studies suggest that stigma and mental health literacy (MHL) are two very important factors that favour this delay and that they vary depending on OCD symptoms. Considering that, the aim of this study is to, firstly, analyze the stigma and OCD literacy in relation to the different obsessive-compulsive symptom contents in the general population and, secondly, assess the impact of an intervention carried out through a mobile application (esTOCma) that has been gamified to decrease stigma and increase OCD literacy. Method. A group of 357 people (39.66 …
Can Brief, Daily Training Using a Mobile App Help Change Maladaptive Beliefs? Crossover Randomized Controlled Trial
BackgroundObsessive-compulsive disorder (OCD) is a disabling condition with a wide variety of clinical presentations including contamination fears, fear of harm, and relationship-related obsessions. Cognitive behavioral models of OCD suggest that OC symptoms result from catastrophic misinterpretations of commonly occurring intrusive experiences and associated dysfunctional strategies used to manage them. OCD-related maladaptive beliefs including inflated responsibility, importance and control of thoughts, perfectionism, and intolerance for uncertainty increase the likelihood of such misinterpretations. ObjectiveConsidering accumulating evidence suggesting that mobile health (mHealth) apps b…
Validation of the Spanish version of the Fear of Self Questionnaire
Abstract Cognitive models, from both the appraisal and inferential confusion perspectives, propose that the self is a relevant variable in the development and maintenance of obsessive-compulsive (OC) disorder. In this study, we examined the psychometric properties of the Spanish version of the Fear of Self Questionnaire (FSQ) and analyzed the role of the fear of self (the sort of person we are afraid of becoming) as a predictor of OC beliefs and symptoms. A sample of 359 non-clinical participants completed a set of questionnaires, including the FSQ. Confirmatory factor analysis replicated the original one-factor solution for both the FSQ-8- and 20-item versions. The FSQ demonstrated excelle…
Contribution of executive functions to eating behaviours in obesity and eating disorders.
AbstractBackground:Patients with eating disorders (ED) or obesity show difficulties in tasks assessing decision-making, set-shifting abilities and central coherence.Aims:The aim of this study was to explore executive functions in eating and weight-related problems, ranging from restricting types of ED to obesity.Method:Two hundred and eighty-eight female participants (75 with obesity; 149 with ED: 76 with restrictive eating, 73 with bingeing-purging symptoms; and 64 healthy controls) were administered the Wisconsin Card Sorting Test, the Iowa Gambling Task, and the Group Embedded Figures Test to assess set-shifting, decision-making and central coherence, respectively. Results:Participants wi…
Clinical validation of a virtual environment for normalizing eating patterns in eating disorders
The purpose of the present study was to examine the clinical validation of a Virtual Reality Environment (VRE) designed to normalize eating patterns in Eating Disorders (ED). The efficacy of VR in eliciting emotions, sense of presence and reality of the VRE were explored in 22 ED patients and 37 healthy eating individuals. The VRE (non-immersive) consisted of a kitchen room where participants had to eat a virtual pizza. In order to assess the sense of presence and reality produced by the VRE, participants answered seven questions with a Likert scale (0-10) during the experience, and then filled out the Reality Judgment and Presence Questionnaire (RJPQ) and ITC-Sense of Presence Inventory (I…
Ego-syntonicity and ego-dystonicity of eating-related intrusive thoughts in patients with eating disorders.
Abstract The main objective of the present study was to analyse the role of the ego-dystonicity and ego-syntonicity of eating disorder intrusive thoughts (EDITs) in the genesis and maintenance of eating disorders (EDs). Participants were 98 female patients with EDs, 56 Spanish and 42 English (27.19±9.59 years; body mass index (BMI): 18.72±2.87). All of them completed the eating attitudes test, the Eating Attitudes Test, the Eating Intrusive Thoughts Inventory, the Ego-Dystonicity Questionnaire-Reduced version, and the Ego-Syntonicity Questionnaire. Patients indicated that their EDITs were rational and also undesirable and immoral, suggesting that EDITs are not fully ego-syntonic or ego-dyst…
Enhancing mental health literacy in obsessive-compulsive disorder and reducing stigma via smartphone: A randomized controlled trial protocol.
Obsessive-compulsive disorder (OCD) is a disabling disorder that can be successfully treated. However, individuals with OCD do not seek or delay seeking treatment. This delay may be explained by poor mental health literacy and stigmatizing attitudes toward OCD in community. In order to work on these variables, a gamified mental health mobile application (app) called esTOCma has been developed. The purpose of this study is to describe the protocol for a study to test the efficacy of esTOCma, increasing mental health literacy and help-seeking intention, reducing the stigmatizing attitudes and social distance suffered by people with OCD, as well as the distress associated with obsessive-compul…
The Spanish version of the Obsessive-Compulsive Inventory-Revised (OCI-R): Reliability, validity, diagnostic accuracy, and sensitivity to treatment effects in clinical samples
Abstract This study examines the psychometric properties of the Obsessive-Compulsive Inventory-Revised (OCI-R) in Spanish and Argentinean samples. 90 OCD patients, 31 with non-OCD anxiety disorders and 84 non-clinical individuals completed the Spanish version of the OCI-R along with other OCD, depression, anxiety, and OCD-related cognition measures. The OCI-R showed significant associations with both OCD severity and other OCD symptom measures, beyond the relationships with depression, anxiety, and worry. The questionnaire also differentiated OCD from other anxiety disorders, except on the ordering and hoarding subscales. The relationships between the OCI-R subscales and dysfunctional belie…
Bidimensional Structure of the Orthorexia: Development and Initial Validation of a New Instrument
Are the Motives for Food Choices Different in Orthorexia Nervosa and Healthy Orthorexia?
Recent research points to the bidimensional nature of orthorexia, with one dimension related to interest in healthy eating (healthy orthorexia) and another dimension related to a pathological preoccupation with eating healthily (orthorexia nervosa). Research was needed to provide further support for this differentiation. We examined the food-choice motives related to both aspects of orthorexia. Participants were 460 students from a Spanish university who completed the Teruel Orthorexia Scale and the Food Choice Questionnaire. By means of structural equation modeling, we analyzed the relationship between orthorexia, food-choice motives, gender, body mass index, and age. The motives predictin…
Eating-related Intrusive Thoughts Inventory: exploring the dimensionality of eating disorder symptoms.
The aims of this study were, first, to examine the structure and validity of the Eating-related Intrusive Thoughts Inventory (INPIAS), a self-report questionnaire designed to assess eating disorders related to intrusive thoughts (EDITs), and second, to explore the existence of a continuum ranging from normal to abnormal thought intrusions related to eating, weight, and shape. Participants were 574 (408 women) nonclinical community individuals. Analyses revealed that EDITs can be clustered into three sets: appearance-dieting, need to exercise, and thoughts-impulses related to eating disorders. EDITs' consequences showed a two-factor structure: emotional consequences/personal meaning and tho…
Similarities and differences between eating disorders and obese patients in a virtual environment for normalizing eating patterns.
Virtual reality has demonstrated promising results in the treatment of eating disorders (ED); however, few studies have examined its usefulness in treating obesity. The aim of this study was to compare ED and obese patients on their reality judgment of a virtual environment (VE) designed to normalize their eating pattern. A second objective was to study which variables predicted the reality of the experience of eating a virtual forbidden-fattening food. ED patients, obese patients, and a non-clinical group (N = 62) experienced a non-immersive VE, and then completed reality judgment and presence measures. All participants rated the VE with similar scores for quality, interaction, engagement,…
The Use of a Nonimmersive Virtual Reality Programme in Anorexia Nervosa: A Single Case-Report
Objective: People with anorexia nervosa (AN) experience high levels of fear and anxiety related to eating. The aim of this case report was to describe the use of a virtual reality (VR) programme developed to facilitate exposure to food as a supplement to treatment for a person with AN. Method: A 21-year-old patient with AN was given the VR module in addition to the Maudsley Model of Treatment for Adults with Anorexia Nervosa. Weight, eating disorder symptomatology (EDE-Q) and general psychopathology (DASS) were assessed before and after the module was delivered. Results: At the end of the module, the patient reported lower levels of anxiety, safety behaviours and fears related to food. Both…
EsTOCma: An App to fight against stigma associated with Obsessive-Compulsive Disorder (OCD)
Introduction: Stigmatizing attitudes is a problem associated to mental disorders. In obsessive-compulsive disorder (OCD) stigma has been associated to shame, guilty and delay asking for help. Objective: To present the design of a mobile health application (app), named EsTOCma, with the objective to increase OCD mental health literacy, help seeking attitudes, and reduce stigmatizing/ self-stigmatizing attitudes and social distance associated with OCD in adult population (non-clinical and OCD population and their relatives). Method: Method: The requirements have been developed based on a literature review on mental health antistigma interventions, OCD stigma research, and serious games resear…
Personality Profile in Orthorexia Nervosa and Healthy Orthorexia
Little is known about orthorexia in both its pathological (orthorexia nervosa, OrNe) and adaptive (healthy orthorexia, HeOr) forms. To date, few studies have been carried out to analyze the personality profile associated with orthorexia, and the results have been contradictory. The aim of the present study was to explore the characteristic pathological personality traits associated with OrNe and HeOr. A total of 297 participants (Mage= 30.8 years; 94.6% women) from the general population completed the Teruel Orthorexia Scale (TOS) and Personality Inventory for DSM-5-Short Form (PID-5-SF) questionnaires. Results showed significant low-medium correlations between OrNe and the four personality…
Teachers' Knowledge and Stigmatizing Attitudes Associated With Obsessive-Compulsive Disorder: Effectiveness of a Brief Educational Intervention
Because children and adolescents are vulnerable to developing obsessive-compulsive disorder (OCD), classroom teachers play an important role in the early identification and intervention in students with OCD. The present study aims to explore the recognition of OCD, general knowledge about this disorder, implications in the classroom, and stigmatizing attitudes among teachers, as well as the effectiveness of a brief educational intervention about OCD. Participants (n = 95; mean age = 43. 29 years old; 64.3% female) were primary and secondary school teachers who were randomly assigned to an experimental group or a control group. All of them completed a set of self-report questionnaires, read …
esTOCma, an app developed to dismiss self-stigma and increase mental health literacy about obsessive-compulsive disorder: how does it perform in a clinical sample?
Introduction. Obsessive-compulsive disorder (OCD) is a disabling condition that can be treated successfully. However, individuals with OCD often fail to seek or delay seeking treatment. In order to overcome this gap and increase the intention to seek for help, a gamified mental health mobile application (app) called esTOCma has been developed with a focus on offering information about OCD and on discussing stigmatizing attitudes toward the disease. The general aim of this study is to analyze the impact of the intervention through esTOCma in a clinical OCD sample. The objective of the study was two-fold. First, to analyze if the intervention changes the following variables in a clinical OCD …
Normalizing the eating pattern with virtual reality for bulimia nervosa: a case report
AbstractThe aim of the present study was to analyze the effects of a virtual reality (VR) component as a therapeutic tool to normalize eating patterns, as part of the cognitive-behavioral treatment of a bulimia nervosa (BN) patient. Results indicated that the patient made significant progress with her eating habits; binges and vomits were eliminated completely. Moreover, when the VR started the patient avoided food, especially meat (5 over 5), and it was reduction of this avoidance in the lasts sessions (2 and 3 over 5). It was also observed a reduction in the avoidance and the fear of eating (from 10 and 9 to 6 and 5 respectively) and increased the impulse control (from 2 to 7). The patien…

Study of Obsessive Compulsive Beliefs: Relationship with Eating Disorders
Background: The relationship between Eating Disorders (ED) and Obsessive Compulsive Disorder (OCD) has been extensively studied in the last few years. However, little effort has been devoted to studying the link between these disorders with regard to their distorted beliefs. Aims: The first objective of the study was to analyze the differences in OCD-related beliefs among ED subtypes and the general population, controlling for age, Body Mass Index, and obsessionality. The second objective was to explore which OCD beliefs explain ED symptomatology. Method: Seventy-nine ED patients without OCD comorbidity, divided into diagnostic subtypes, and 50 community participants completed the Obsessive…
Spanish version of the Inferential Confusion Questionnaire-Expanded Version: Further support for the role of inferential confusion in obsessive-compulsive symptoms.
The purposes of this research were (1) to analyse the psychometric properties of the Inferential Confusion Questionnaire-Expanded Version (ICQ-EV) in a Spanish population; (2) to explore the role of inferential confusion in obsessive-compulsive disorder (OCD); and (3) to compare the inferential confusion construct in nonclinical and clinical samples. A sample of 342 nonclinical participants and 66 patients with OCD completed the ICQ-EV Spanish adaptation as well as a set of questionnaires. Results confirmed a good fit of the ICQ-EV Spanish version to the original unifactorial structure and excellent internal consistency and test-retest reliability. Moreover, results confirmed that the ICQ-E…
EsTOCma: A Mental Health Application for Enhancing Mental Health Literacy About Obsessive-compulsive Disorder and Reduce Stigma: Protocol for a Randomized Controlled Trial.
Introduction: Obsessive-compulsive disorder (OCD) is a debilitating heterogeneous condition and one of the 10 leading causes of disability worldwide, affecting approximately 2%-2.3% of the population. However, there is a long delay in seeking treatment, and between 38% and 89.8% of OCD sufferers neither ask for nor receive treatment for their symptoms. Studies on OCD report that insufficient mental health literacy about OCD and the associated stigma would explain the delayed treatment-seeking behavior of OCD patients. With the aim of increasing OCD mental health literacy and reducing the associated stigma, a gamified mental health mobile application (app) called esTOCma was developed. The o…
Modelo de personalidad HEXACO: Relaciones con psicopatología emocional en una muestra española [HEXACO personality model: Relationships with emotional psychopathology in a Spanish sample]
This study examines the relationships between personality dimensions and obsessive-compulsive and depressive psychopathology signs, on the basis of the new 6-dimension HEXACO personality model:Honesty-Humility, Emotionality, eXtraversion, Agreeableness, Conscientiousness, and Openness to Experience. Participants were 354 individuals from the general population, from which 21 were at risk of depression (r-DEP) and 29 at risk of obsessive-compulsive disorder (r-OCD). Jointly, the 6 personality dimensions explained 42% of the BDI-II score in the r-DEP group, and 37% of the C-BOCI score in the r-OCD group. The two at-risk groups differed from the no-risk participants in all the personality dime…
Analysis of the Effectiveness of Cognitive Remediation and Emotional Skills Training in a Group format. Preliminary results in patients with eating disorders
Eating disorders (ED) are associated with cognitive and emotional impairments. Cognitive and Emotional Remediation Skill Training (CREST) was developed as an intervention program targeting patients' thinking styles and their skills in recognizing and managing emotions. AIM: to analyze the effects of the CREST intervention in a group format in females with ED. METHOD: eight females underwent a CREST program (eight 90-minute sessions) targeting cognitive (central coherence, set shifting, problem solving) and emotional (recognizing emotions, managing emotions) difficulties. It was assessed cognitive flexibility (Wisconsin Card Sorting Test), decision-making (Iowa Gambling Task), central cohere…
Orthorexia nervosa and healthy orthorexia as new eating styles
It was recently proposed that healthy orthorexia (HeOr) and orthorexia nervosa (OrNe) should be differentiated. The aim of the present study was to analyze whether the two dimensions of orthorexia can be considered new eating styles or basically equivalent to restrained eating behavior. Two samples of university students (sample 1, n = 460; sample 2, n = 509) completed the Teruel Orthorexia Scale (TOS), the Dutch Eating Behavior Questionnaire (DEBQ), and the Positive and Negative Affect Schedule (PANAS). Factor analysis with the TOS and DEBQ items together revealed an adequate fit for the preexisting five-factor solution (TOS: OrNe and HeOr; DEBQ: Restrained Eating, Emotional Eating, and Ex…
What do Spanish adolescents think about obsessive-compulsive disorder? Mental health literacy and stigma associated with symmetry/order and aggression-related symptoms.
Obsessive-compulsive disorder (OCD) is a frequent and disabling disorder with a long delay in seeking help that could partly be due to poor mental health literacy and stigmatizing attitudes. This study analyzes the mental health literacy and stigma associated with symmetry/order and aggression-related OCD in a Spanish adolescent sample. This age group was chosen because adolescence is a vulnerable period for the development of OCD, and adolescents are often reluctant to seek professional help. One hundred and two non-clinical adolescents read two vignettes describing symmetry/order and aggression-related OCD. Then, referring to these two vignettes, they answered questions related to problem…
Obsessional and Eating Disorder-related Intrusive Thoughts: Differences and Similarities Within and Between Individuals Vulnerable to OCD or to EDs
Unwanted intrusive cognitions constitute the normal variant of clinically significant intrusive cognitions found in disorders such as obsessive-compulsive disorder (OCD) and eating disorders (EDs). This study investigates whether individuals who are vulnerable to OCD or EDs experience more intrusions than people with no vulnerability to these disorders, and it examines the consequences of obsessional (OITs) and eating disorder (EDITs) intrusions in the same individuals, taking into account their susceptibility to OCD, EDs or neither of the two. From a sample of 922 participants, three groups were formed: risk of OCD (n = 92), risk of EDs (n = 41) and a no-risk group (n = 100). EDITs were mo…
Measuring Orthorexia Nervosa: Psychometric Limitations of the ORTO-15.
AbstractOrthorexia nervosa has recently been defined as excessive preoccupation with healthy eating, causing significant nutritional deficiencies and social and personal impairments. The ORTO-15 is the most widely used instrument to evaluate orthorexia nervosa, although previous studies obtained inconsistent results about its psychometric properties, and there are no data on the Spanish version. Thus, the main objective of the present study was to analyze the psychometric properties of the Spanish adaptation of the ORTO-15. In order to cross-validate the results, two independent samples were used (Sample 1: n = 807, 74.1% women; Sample 2: n = 242, 63.2% women). The results did not support t…
Assisting relapse prevention in OCD using a novel mobile app-based intervention: A case report.
Mobile health apps increase the accessibility of cognitive-behavioral–based interventions before, during, or following treatment. GGOC is a mobile app designed to challenge maladaptive beliefs in obsessive-compulsive disorder (OCD). This single-case study assesses the usefulness of GGOC as a relapse prevention tool for OCD. The patient was a 26-year-old woman with severe contamination and washing/cleaning OCD symptoms (Y-BOCS = 33). GGOC was used for relapse prevention following CBT treatment. The patient completed 47 levels dedicated to OCD-relevant maladaptive beliefs. Before and after GGOC, the Y-BOCS, Obsessive-Compulsive Inventory (OCI-R), Obsessive Beliefs Questionnaire (OBQ-20), and…
A novel approach to challenging OCD related beliefs using a mobile-app: An exploratory study
Abstract Background and objectives According to cognitive models, obsessive compulsive symptoms result from catastrophic misinterpretations of commonly occurring intrusive experiences and the use of counterproductive strategies to manage them. Obsessive Compulsive Disorder (OCD) related beliefs such as inflated responsibility, importance of thoughts and perfectionism increases the likelihood of such misinterpretations. Consistent with a growing body of literature supporting the usefulness of mobile delivered technologies in fostering cognitive behavior change, the present study assessed the effectiveness of a novel cognitive training exercise designed to challenge OCD-related beliefs. This …
Confirmatory factor analysis and psychometric properties of the Spanish version of the Multidimensional Body-Self Relations Questionnaire-Appearance Scales in early adolescents
The main aim of this study was to confirm the factorial structure of the Spanish version of the Multidimensional Body-Self Relations Questionnaire-Appearance Scales in early adolescents from 12 to 14 years. The sample included 355 participants, 189 girls and 166 boys, with ages ranging from 12 to 14 years old. The original MBSRQ-AS 5-factor structure was confirmed, and the model showed a good fit to the data: Appearance Evaluation, Appearance Orientation, Body Areas Satisfaction, Overweight Preoccupation, and Self-Classified Weight. The internal consistency of the test scores was adequate. Girls had higher score s than boys on Appearance Orientation, Overweight Preoccupation, and Self-Class…
