Die Mukoviszidose (zystische Fibrose) ist eine autosomal rezessiv vererbbare Krankheit mit einer Inzidenz von ca. 1:2500 Neugeborene. Bis vor 20 Jahren verstarb die uberwiegende Mehrzahl der Patienten in der fruhen Kindheit. Seither hat sich die Prognose durch eine konsequente Dauertherapie in spezialisierten Zentren deutlich verbessert. Die mittlere Lebenserwartung liegt in Deutschland und den USA derzeit bei etwa 41°Jahren. Somit werden Anasthesist und Intensivmediziner zunehmend mit Erwachsenen konfrontiert, die an Mukoviszidose leiden.

Die Porphyrien sind eine Gruppe vorwiegend hereditarer Stoffwechselstorungen der Hambiosynthese. Die Betreuung eines Patienten mit akuter Porphyrie ist eine interdisziplinare Herausforderung, bei der die Anasthesie eine koordinierende Funktion erfullt. Da es fur die Porphyrien bislang keine kausale Therapie gibt, richtet sich das Vorgehen im akuten Schub nach dem klinischen Bild. Im anasthesiologischen Umfeld ist insbesondere auf die Verwendung „sicherer“ Medikamente zu achten!
