Acute pancreatitis in children and rotavirus infection. Description of a case and minireview

In this article, we describe a case of acute pancreatitis occurred on a 2-year-old boy following rotavirus gastroenteritis. Its characteristics are analyzed and contextualized among those of other 4 cases of pancreatitis associated with rotavirus infection found through a systematic review of the international literature. None of the five children underwent surgery or was referred to an intensive care unit and all the 5 children cured with normalization of pancreatic enzyme within 5-10 days. The pathogenesis of this rare complication remains to be cleared, and its actual incidence may be higher than that reported. Although acute pancreatitis associated with rotavirus gastroenteritis seems t…

The first coronal mass ejection observed in both visible-light and UV HI Ly-α channels of the Metis coronagraph on board Solar Orbiter

Context. The Metis coronagraph on board Solar Orbiter offers a new view of coronal mass ejections (CMEs), observing them for the first time with simultaneous images acquired with a broad-band filter in the visible-light interval and with a narrow-band filter around the H ILy-αline at 121.567 nm, the so-called Metis UV channel.Aims.We show the first Metis observations of a CME, obtained on 16 and 17 January 2021. The event was also observed by the EUI/FSI imager on board Solar Orbiter, as well as by other space-based coronagraphs, such as STEREO-A/COR2 and SOHO/LASCO/C2, whose images are combined here with Metis data.Methods.Different images are analysed here to reconstruct the 3D orientation…

The return of anthrax. From bioterrorism to the zoonotic cluster of Sciacca district [Il ritorno del carbonchio. Dal bioterrorismo al focolaio zoonotico del distretto di Sciacca]

Anthrax is a disease caused by Bacillus anthracis which affects herbivorous animals. Humans acquire the disease incidentally by exposure to infected animals, animal products or spores on soil. The infection is still endemic in many regions in developing countries. In Italy animal clusters are very rare and human cases are exceptional. Bacillus anthrax is also a potential source for acts of bioterrorism. In the natural human infection, cutaneous anthrax is the most widespread, while the other two, pulmonary and gastrointestinal anthrax, are very rare forms. We describe the first case of human anthrax occurring in western Sicily in the last twenty years. The cutaneous lesion healed without si…

Pulmonary hydatid disease in childhood: a case report [Idatidosi polmonare in età infantile: Descrizione di un caso clinico]

Echinococcosis/hydatidosis is a parasitic zoonosis caused in humans by the larval stage of the Echinococcus granulosus cestode. Although notification of infestation is required, there are no reliable data on the prevalence of this parasitosis among humans in Italy. This zoonosis was first reported in ancient times (Hippocrates, fourth century BC) and since then many cases of echinococcosis/hydatidosis have been described. Currently, it is considered one of the main parasitoses affecting animals bred for income and one of the most important parasitic zoonoses. We present the clinical case of a three-year-old child, living in Palermo with a dog in his house, who came to our observation for dy…

Coronary involvement in Mediterranean spotted fever

Mediterranean spotted fever (MSF) is a tick-borne acute febrile disease caused by Rickettsia conorii and characterized by fever, a maculo-papular rash and a black eschar at the site of the tick bite. We describe the case of a 3-year-old boy with MSF who developed a transient right coronary artery ectasia. The patient was brought to the hospital after four days of fever and mild myalgia of the legs. The suspicion of MSF arose due to the presence of a maculo-papular skin rash and treatment with oral clarithromycin was started. After four days fever persisted and the differential diagnosis of Kawasaki syndrome was considered. Echocardiography showed a dilated right coronary artery with hyperre…

COVID-19 temporally related multisystem inflammatory syndrome (MIS-C): an early window of opportunity is a good treatment strategy? The experience of the paediatric covid center in Palermo

Introduction: Multi-system inflammatory syndrome in children (MISC) shows a presentation mimicking Kawasaki Disease (KD), Toxic Shock Syndrome (TSS), Macrophage Activation Syndrome (MAS). Furthermore, many children show respiratory or abdominal symptoms. Objectives: Intravenous immunoglobulin (IVIG) is recommended as first line treatment as in KD, followed by aspirin, steroids and, in IVIGresistant patients, IL-1 or IL-6 blocking agents. Methods: We describe a cohort of 16 Sicilian children (6M;10F; age:1.4-14 years), with MIS-C, with clinical features compatible with classical or incomplete KD, in some cases with MAS and/or TSS. Demographic, clinical, laboratory, echocardiographic and imag…

Spider bite mimicking pyoderma gangrenosum in sJIA: a case report

Introduction: Several paediatric patients manifest conditions commonly misdiagnosed as spider bites, which however, can include other arthropods bites; bacterial, viral, and mycotic infections; vasculitis; dermatological diseases; miscellaneous conditions as drug reactions, chemical injuries. Objectives: In Italy, spiders which are likely to be associated with severe toxin mediated tissue damage are uncommon, especially in urban zones. However, a minor trauma may be a precipitating factor for pyoderma gangrenosum particularly over the legs, in association with inflammatory bowel disease, haematologic diseases and Juvenile Idiopathic Arthritis (JIA).

[Anti-Pseudomonas aeruginosa antibodies and lung disease in cystic fibrosis]

The aim of our study was to diagnose and to control three aspects of the evolution of lung disease in CF: the absence of infection, the intermittent colonization and chronic infection by Pseudomonas aeruginosa. Therefore a study of anti-pseudomonas antibodies (Ab) (anti-protease, anti-elastin and antihexo-toxin A) for diagnosis and follow-up of CF patients was considered. Moreover, we related the presence of Ab to the sputum culture, to FEV1, to patient age and to genotype. Tbe Ab were dosed in 121 patients by quantitative ELISA method. Values < 1: 500 were considered negative, values> 1: 500 and < 1:1250 borderline, and > 1:1250 positive. 16.5% of patients did not have Ab, 17% …
