Intensity-modulated radiation therapy, image-guided radiation therapy with fiducial markers and prostate brachytherapy allow the delivery of dose escalation for localized prostate cancer with very low rates of long-term toxicity and sequelae. Resume La prostatectomie est le traitement de reference du cancer localise de la prostate. Depuis 2004 le Robot Da Vinci ® est utilise par notre centre dans le cadre de la prostatectomie totale laparoscopique. Objectif Evaluer l’interet de la laparoscopie robot-assistee (PTLR) dans la prostatectomie totale en la comparant a la laparoscopie standard (PTL) sur les resultats fonctionnels urinaires et carcinologiques a trois ans de recul. Materiel Deux cent quarante-sept patients ont ete operes de mars 2004 a avril 2007, dont 125 en PTL et 122 en PTLR, de facon consecutive et de maniere sequentielle, par un seul chirurgien senior au debut de son experience.
