Asthma-COPD Overlap Syndrome (ACOS) has been recently defined as a new pathological entity. Most studies support a large difference in the pathophysiology of bronchial asthma and chronic obstructive pulmonary disease (COPD). However, there is evidence of an increasing prevalence of patients in whom the two diseases coexist or in which one condition evolves into the other, leading to the pathological condition named ACOS. This occurs mainly in individuals with long-standing asthma, especially if also current or former-smokers. Bronchial hyperreactivity seems to induce a loss of the deep inspiration's bronchoprotective effect which is normally able to dilate the obstructed airways. Studies such as the HRCT on the bronchial gauge's alterations show that the deep inspirations bronchodilator effect is associated with the bronchial distensibility. In fact, it is almost totally absent either in the elderly or in the COPD patients. In asthma, instead, the pathology's seriousness has proved to be associated with the impairment of the deep inspiration's broncho-protective effect which this way becomes an important diagnostic parameter.
Effects of rowing training on airway responsiveness in asthmatics
We previously reported that after a short course of rowing training the responsiveness to methacholine (Mch) in the absence of deep inspiration (DI) decreased in healthy subjects. We assessed whether a similar beneficial effect of exercise on airway responsiveness could occur in asthmatics. Nine patients (M/F: 3/6; mean age±SD: 24±2 yrs) with mild untreated asthma (FEV(1): 100±7.4% pred, FEV(1)/VC 90±6.5%) underwent a series of single dose Mch bronchoprovocations in the absence of DI in the course of a indoor rowing training consisted of two to three sessions per week for 10 weeks (total 28 sessions).
