PURPOSE: The goals of transurethral resection of a bladder tumor (TUR) are to completely resect the lesions and to make a correct diagnosis to adequately stage and treat the patient. Persistent disease after TUR is not uncommon and is why re-TUR is recommended in T1G3 patients. When there is T1 tumor in the re-TUR specimen, very high risks of progression (82%) have been reported. We analyze the risks of recurrence, progression to muscle-invasive disease and cancer-specific mortality (CSM) according to tumor stage at re-TUR in T1G3 patients treated with BCG. ABH-antigenicity of transitional cell carcinoma of the urinary bladder in patients subjected to topical chemoprophylaxis

Topical chemoprophylaxis with adriamycin was administered to 49 patients with UICC-stages pTA and PT1 (pathological stage 0 and A) of transitional cell carcinoma of the urinary bladder. Tumor specimens obtained prior to initiation of the instillation protocol were evaluated with regard to ABH-antigenicity. ABH-antigens were present in tumors of 19 patients and none of these developed muscle-infiltrating recurrences during or after intravesical chemoprophylaxis. The primary lesions of 30 patients were deleted of antigens; only 5 (16%) developed a progressive recurrence. A total of 62 patients at high risk for recurrence of superficial bladder cancer were selected for a study designed to compare the prophylactic efficacy of different doses and schedules of sequential intravesical instillations of epirubicin and interferon-alpha-2b and to evaluate which sequence could enhance the release of cytokines in the urine. Our investigations showed a significant increase in urinary concentrations of interleukins in patients who received the sequential intravesical administration of epirubicin and interferon-alpha-2b. Introduction & Objectives: Up to 70% of patients affected by intermediate risk NMI-BC recur after intravesical therapy (IT). The majority of them will be retreated by IT. The therapeutic strategy for these patients is not well defined. BCG is advocated when intravesical chemotherapy (ICH) fails. A pilot study in 62 patients has been carried out to evaluate the combination of epirubicin and interferon (IFN)-alpha 2b using different doses and schedules of intravesical administration in the prevention of recurrence of bladder cancer. Preliminary results show greater prophylactic efficacy not only when higher doses of epirubicin and IFN-alpha 2b are used but also when a larger interval between instillation of the compounds was used.

Large cooperative trials are more likely than series studied by small groups to bring about significant progress in the field of intravesical adjuvant chemotherapy of superficial bladder tumor. Multicenter randomized trials involving large numbers of patients have been conducted in Europe by the EORTC Urological Group. The Group's main objectives were to compare the efficacy of thio-TEPA, VM-26, epodyl, Adriamycin, and cisplatin, against no treatment, and to study the prophylactic effect of oral pyridoxine and evaluate the main prognostic factors. The role of chemotherapy in the management of transitional bladder tumors can conveniently be considered under two principal headings — topical (intracavitary) and sytemic chemotherapy, as the indications for these modalities are distinctly different. The intent of topical administration of cytoxic agents may be (1) prophylactic to prevent recurrences or new occurrences after TUR; (2) curative to clear the bladder of extensive non-resectable multifocal disease, or to control carcinoma in situ.
