Oral myiasis in mentally challenged patient: a case report

Myiasis is the infestation of live human and vertebrate animal with Dipterous larvae which feed on host’s dead or living tissue. This condition is a result of neglected approach towards oral hygiene along with mental and physical challenges of patient. A case of oral myiasis in the maxillary anterior region in an 18 years old female with neurologic defi-cit caused by the larvae (maggots) is reported. The management consists of manual removal of larvae by topical application of turpentine oil, oral therapy with Ivermectin and surgical debridement of oral wound.

Treacher Collins syndrome (TCS) or Franceschetti syndrome is an autosomal dominant disorder of craniofacial development with variable expressivity. It is named after E Treacher Collins who described the essential components of the condition in 1900. Incidence of this syndrome is approximately 1 in 50,000 live births and it affects both genders equally. It affects structures which are derivatives of the first and second brachial arches. Drooling is the uncontrolled leakage of saliva outside the mouth, generally as a result of difficulty in swallowing the saliva produced. Many factors contribute to drooling, though it is more commonly seen in children with brain paralysis - particularly those receiving anticonvulsivant medication. Drooling is also often seen in patients with lip sealing problems or malocclusions such as anterior open bite. Clinically, the affected patients can develop skin irritation or abrasions, problems of hygiene, unpleasant smell and - in the more severe presentations - the need to wear protectors or frequently change clothing. When the management of hemostasis is inadequate, these patients are at high risk of experiencing intra and postoperative bleedings after tooth extractions.
