The diagnostic potential of Fe lines applied to protostellar jets

We investigate the diagnostic capabilities of the iron lines for tracing the physical conditions of the shock-excited gas in jets driven by pre-main sequence stars. We have analyzed the 300-2500 nm X-shooter spectra of two jets driven by the pre-main sequence stars ESO-Halpha 574 and Par-Lup 3-4. Both spectra are very rich in [FeII] lines over the whole spectral range; in addition, lines from [FeIII] are detected in the ESO-H\alpha 574 spectrum. NLTE codes along with codes for the ionization equilibrium are used to derive the gas excitation conditions of electron temperature and density, and fractional ionization. Evidence for extrinsic control of oesophageal primary peristalsis.

AbstractThe role of both peripheral and central mechanism in the control of primary peristalsis was studied in pigeon cervical oesophagus.The results from the transection of oesophageal muscular wall and of extrinsic nerves suggest that :i) primary peristalsis is programmed centrally.ii) extrinsic motor input is carried in glossopharyngeal nerves and distributed separately at each oesophageal level through intramural neurons.iii) intramural neurons do not seem capable of propagating the peristaltic sequence irrespective of the central control.

Abstract Zollinger-Ellison syndrome is an often progressive, persistent and frequently life-threatening disease, described for the first time as characterized by ulceration of the upper jejunum, hypersecretion of gastric acid and non-beta islet cell tumors of the pancreas; this syndrome is due to the hypersecretion of gastrin. We report a case of Zollinger-Ellison syndrome presenting as severe esophagitis evolving in stenosis, which demonstrates how a delayed diagnosis may induce risk of disease spreading. AbstractRobotic surgery is gaining importance in complex thoracoscopic surgery, such as robotic-assisted minimally invasive esophagectomy (RAMIE). The RAMIE procedure was designed using the first generation of the robotic system. The latest da Vinci Xi system has substantially increased the dexterity, especially designed for multiquadrant surgery. The original three-arm RAMIE approach was modified including the robotic four-arm use for the thoracoscopic and laparoscopic part of the operation. This extended approach (four-arm RAMIE approach) provides more flexibility and raises the independence of the surgeon.

In a real-life cohort of patients with AHRF undergoing HFNO, ΔPnose showed excellent accuracy in predicting early failure, similar of that displayed by ROX. Giving that the decision to upgrade to NIV or MV was based on clinical variables, the high accuracy of ROX in predicting failure of HFNO is not surprising. The similar accuracy of ΔPnose (the only measurement that remained blinded to our clinical decision), strengthened the association with outcome, avoiding incorporation bias.

Portal vein thrombosis may occur in cirrhosis; nevertheless, its prevalence, and predictors are still elusive. To investigate this issue, the Italian Society of Internal Medicine undertook the “Portal vein thrombosis Relevance On Liver cirrhosis: Italian Venous thrombotic Events Registry” (PRO-LIVER). This prospective multicenter study includes consecutive cirrhotic patients undergoing Doppler ultrasound examination of the portal area to evaluate the prevalence and incidence of portal vein thrombosis over a 2-year scheduled follow-up.
