Multidisciplinary system for detecting medication errors in antineoplastic chemotherapy.

Objective. To analyze medication errors (MEs) in a multidisciplinary system with a Computerized Pharmacotherapy Process (CPP) in cancer patients. Design. A longitudinal, prospective 2-year (January 2003 —to December 2004) cohort study was made in adult patients administered antineoplastic treatment in Services of Oncology and Haematology. Common and uncommon pitfalls in pancreatic imaging: it is not always cancer.

Despite advances in multimodality imaging of pancreas, there is still overlap between imaging findings of several pancreatic/peripancreatic disease processes. Pancreatic and peripancreatic non-neoplastic entities may mimic primary pancreatic neoplasms on ultrasound, CT, and MRI. On the other hand, primary pancreatic cancer may be overlooked on imaging because of technical and inherent factors. The purpose of this pictorial review is to describe and illustrate pancreatic imaging pitfalls and highlight the basic radiological features for proper differential diagnosis.

A Review of Healthcare Failure Mode and Effects Analysis (HFMEA) in Radiotherapy

This paper presents a review of risk analyses in radiotherapy (RT) processes carried out by using Healthcare Failure Mode Effect Analysis (HFMEA) methodology, a qualitative method that proactively identifies risks to patients and corrects medical errors before they occur. This literature review was performed to provide an overview of how to approach the development of HFMEA applications in modern RT procedures, comparing recently published research conducted to support proactive programs to identify risks. On the basis of the reviewed literature, the paper suggests HFMEA shortcomings that need to be addressed.

Purpose To assess visual and refractive results after bilateral implantation of a trifocal intraocular lens (IOL) in patients with hyperopia. Methods In a retrospective nonrandomized study, 196 eyes of 98 patients had bilateral implantation of a trifocal IOL. The Barrett Universal II formula was used for IOL power calculation. Eyes were divided into two groups for their analysis: low-moderate, with IOL power ranging from 22 to 26 D, and high, with IOL power ranging from 25 to 34 D. Patienten und Methode. Von Dezember 1996 bis Februar 1999 wurde bei insgesamt 44 phaken Augen von 28 Patienten eine Worst Iris Claw Lens implantiert.
