Current regimen for high-grade gliomas is maximal safe surgical resection followed by external beam radiotherapy with concurrent temozolamide. Maximal tumor resection, however, must be balanced with preservation of the patient’s neurological function. A crucial prognostic factor in oncological neurosurgery is the extent of resection. Several studies have addressed the importance of extent of resection in gliomas surgery. Despite development in the fields of pre operative and intraoperative neuroimaging and neuromonitoring have ameliorated the survival rate and the quality of life for patients affected by high grade gliomas, the clinical outcome of patients with such gliomas remains extremel…

The impact of tumor nitric oxide production on VEGFA expression and tumor growth in a zebrafish rat glioma xenograft model.

2015

International audience; To investigate the effect of nitric oxide on tumor development, we established a rat tumor xenograft model in zebrafish embryos. The injected tumor cells formed masses in which nitric oxide production could be detected by the use of the cell-permeant DAF-FM-DA (diaminofluorophore 4-amino-5-methylamino-2'-7'-difluorofluorescein diacetate) and DAR-4M-AM (diaminorhodamine-4M). This method revealed that nitric oxide production could be co-localized with the tumor xenograft in 46% of the embryos. In 85% of these embryos, tumors were vascularized and blood vessels were observed on day 4 post injection. Furthermore, we demonstrated by qRT-PCR that the transplanted glioma ce…

End-of-Life Care in High-Grade Glioma Patients. The Palliative and Supportive Perspective.

2018

High-grade gliomas (HGGs) are the most frequently diagnosed primary brain tumors. Even though it has been demonstrated that combined surgical therapy, chemotherapy, and radiotherapy improve survival, HGGs still harbor a very poor prognosis and limited overall survival. Differently from other types of primary neoplasm, HGG manifests also as a neurological disease. According to this, palliative care of HGG patients represents a peculiar challenge for healthcare providers and caregivers since it has to be directed to both general and neurological cancer symptoms. In this way, the end-of-life (EOL) phase of HGG patients appears to be like a journey through medical issues, progressive neurologic…

Conventional MRI-Derived Biomarkers of Adult-Type Diffuse Glioma Molecular Subtypes: A Comprehensive Review.

2022

The introduction of molecular criteria into the classification of diffuse gliomas has added interesting practical implications to glioma management. This has created a new clinical need for correlating imaging characteristics with glioma genotypes, also known as radiogenomics or imaging genomics. Although many studies have primarily focused on the use of advanced magnetic resonance imaging (MRI) techniques for radiogenomics purposes, conventional MRI sequences remain the reference point in the study and characterization of brain tumors. A summary of the conventional imaging features of glioma molecular subtypes should be useful as a tool for daily diagnostic brain tumor management. Hence, t…

Brain mapping as helpful tool in brain glioma surgical treatment—Toward the “perfect surgery”?

2018

Gliomas are the most common primary malignant brain tumours in adults, representing nearly 80%, with poor prognosis in their high-grade forms. Several variables positively affect the prognosis of patients with high-grade glioma: young age, tumour location, radiological features, recurrence, and the opportunity to perform post-operative adjuvant therapy. Low-grade gliomas are slow-growing brain neoplasms of adolescence and young-adulthood, preferentially involving functional areas, particularly the eloquent ones. It has been demonstrated that early surgery and higher extent rate ensure overall longer survival time regardless of tumour grading, but nowadays, functional preservation that is as…

New Hope in Brain Glioma Surgery: The Role of Intraoperative Ultrasound. A Review

2018

Maximal safe resection represents the gold standard for surgery of malignant brain tumors. As regards gross-total resection, accurate localization and precise delineation of the tumor margins are required. Intraoperative diagnostic imaging (Intra-Operative Magnetic Resonance-IOMR, Intra-Operative Computed Tomography-IOCT, Intra-Operative Ultrasound-IOUS) and dyes (fluorescence) have become relevant in brain tumor surgery, allowing for a more radical and safer tumor resection. IOUS guidance for brain tumor surgery is accurate in distinguishing tumor from normal parenchyma, and it allows a real-time intraoperative visualization. We aim to evaluate the role of IOUS in gliomas surgery and to ou…

Innovation in Neurosurgery: Integration Between Cutting-Edge Devices and "Old-Fashioned" Surgical Technique.

2019

n times of rapid scientific and technological progress, doctors must be able to pursue innovations. However, although clinical decision making can rely on new opportunities, the optimism should be tempered by equally newfangled limits, mainly of economic nature, that can bind medical management. The 3 cornerstones of medical progress are genomics, nanotechnologies, and robotics, the future of which lie in the integration among imaging, digital telemedicine, and artificial intelligence. With the aid of wireless and mobile technologies, the world of digital medicine is growing quickly. In this scenario, the simple control of vital signs no longer needs spatial relationship; the continuous con…

Fluorescein for resection of high-grade gliomas: A safety study control in a single center and review of the literature

2017

Background: The importance of a complete resection of high-grade gliomas (HGGs) has been highlighted in scientific literature, in order to limit tumor recurrence and above all to improve disease-free survival rates. Several fluorescent biomarkers have been tested to improve intraoperative identification of residual tumor; 5-aminolevulinic acid (5-ALA) and fluorescein sodium (FS) are now starting to play a central role in glioma surgery. We performed a retrospective analysis on 47 patients operated for HGGs. Here we report our preliminary data. Methods: Data of 47 consecutive patients with HGG have been collected in our study (25 males, 22 females; mean age: 60.3 years, range: 27-86 years). …
