The evaluation of a new diagnostic or imaging device has to focus both on its diagnostic validity and reliability. Cohen's kappa coefficient can be used to assess the agreement of the new device's findings with those of an established diagnostic reference, it therefore presents a surrogate measure of validity. Additional measures of validity are sensitivity and specificity, which assess the order of agreement with the reference for positive and negative reference findings, respectively. The latter, however, can only be applied for binary reference findings. If discordant findings are observed, the McNemar test can be used to detect a diagnostic shift between diagnostic novum and reference. …

BACKGROUND: Data are lacking on the long-term effect on cardiovascular events of adding sitagliptin, a dipeptidyl peptidase 4 inhibitor, to usual care in patients with type 2 diabetes and cardiovascular disease. METHODS: In this randomized, double-blind study, we assigned 14,671 patients to add either sitagliptin or placebo to their existing therapy. Open-label use of antihyperglycemic therapy was encouraged as required, aimed at reaching individually appropriate glycemic targets in all patients. The antiphospholipid syndrome (APS) was described in 1983 as a clinical entity characterized by venous and arterial thrombosis, thrombocytopenia, and recurrent fetal loss. The serological markers of APS are antiphospholipid antibodies (APLA) directed mainly against anionic phospholipids, usually cardiolipin but also phosphatidylserine.
