After a largely inconspicuous pregnancy, a 31-year old primipara suffered from a status epilepticus in the third trimenon. The convulsions could not be terminated by emergency medical services, resulting in aspiration of gastric contents. Assuming eclampsia, an emergency caesarean section was performed immediately in a central hospital. Postoperatively, a pathological pattern of tendon reflexes was noticed. A CT scan revealed subarachnoid haemorrhage. The causal aneurysm of the right A. pericallosa was clipped subsequently. Eclampsia is the leading cause of epileptic seizures during pregnancy. A randomized controlled trial of intervention in fear of childbirth*1

Abstract OBJECTIVE: To compare intensive and conventional therapy for severe fear of childbirth. METHODS: In Finland, 176 women who had fear of childbirth were randomly assigned at the 26th gestational week to have either intensive therapy (mean 3.8 ± 1.0 sessions with obstetrician and one with midwife) or conventional therapy (mean 2.0 ± 0.6 sessions), with follow-up 3 months postpartum. Pregnancy-related anxiety and concerns, satisfaction with childbirth, and puerperal depression were assessed with specific questionnaires. MicroRNAs of the miR-302 cluster are involved in early embryonic development and somatic cell reprogramming. Expression of the miR-302 gene is regulated by the binding of the pluripotency factors Oct4, Sox2 and Nanog to the miR-302 promoter. The specific expression pattern of the miR-302 gene suggested that additional transcription factors might be involved in its regulation. Alpha-adrenergic drugs in retrograde ejaculation.

6 men with complete or partial loss of ejaculation as a result of retroperitoneal lymphadenectomy (performed because of testicular tumor) were treated with an intravenous injection of 60 mg synephrine. Synephrine stimulates adrenergic alpha-receptors. Only 1 man with sustained retrograde ejaculation improved following treatment showing antegrade ejaculation. This temporary restoration of fertility was thought to be due to an increase in bladder neck tone and prevention of backflow of semen into the bladder as a result of stimulation of adrenergic alpha-receptors.
