Poor Esophageal Motility: A Tailored Approach?

New onset dysphagia following antireflux surgery is among the most undesirable side effects of an otherwise excellent therapy. While its cause is multifactorial, insufficient circular muscle strength of the esophageal body, not powerful enough to force the bolus through the distal neo-high pressure zone, can be a component of the pathophysiology. The relative merits of “tailoring” the degree of fundoplication based upon esophageal body motility and/or other clinical features have been debated for decades. Herein we discuss the rationale for a tailored approach, its pros and cons and review data published to date available to guide the clinician in individual patient decision-making.

Objective: To investigate the effects of 4-months intensive aquatic resistance training on body composition and walking speed in post-menopausal women with mild knee osteoarthritis (OA), immediately after intervention and after 12-months follow-up. Additionally, influence of leisure time physical activity (LTPA) will be investigated. Design: This randomised clinical trial assigned eighty-seven volunteer postmenopausal women into two study arms. La implantación y difusión de las artes marciales japonesas en Occidente supuso la importación de prácticas socioculturales privadas de las premisas de su contexto de origen. ¿Cómo afrontar estas prácticas que son ajenas a nuestra identidad cultural? Una interpretación estricta exige entenderlas como prácticas corporales con una idiosincrasia cultural, caracterizadas por incorporar una experiencia vivencial en la que el cuerpo ocupa un lugar preeminente. Su enseñanza y aprendizaje se establecen a través de un sistema triangular integrado por una esencia espiritual, una forma técnica y una estructura física (sin-gi-tai), que se inserta en un proceso singular (shu-ha-ri).
Uveal effusion syndrome complicated by anterior ischemic optic neuropathy

We report on a case of idiopathic uveal effusion syndrome complicated by AION. To our knowledge such an association hasn't been previously described. We suggest that scleral thickening caused obstruction of vortex veins followed by uveal effusion and compression of posterior ciliary arteries within their intrascleral tract, leading to AION. Nevertheless it can't be excluded that AION was the result of mechanical compression on ciliary vessels of optic disc by choroidal detachment. © 1996, Kluwer Academic Publishers. All rights reserved.

Abstract The present study examined whether structural peculiarities in the brain-efferent pathway to the organ of Corti may underlie functional differences in hearing between pigmented and albino individuals of the same mammalian species. Several Usher syndrome (USH)-associated proteins are known to localize to the connecting cilium of photoreceptor cells. The unconventional myosin MYO7A (USH1B) was long accepted as the transport molecule responsible for the ciliary localization of USH proteins. However, based on the typical location of several of the USH proteins along the ciliary axoneme, the involvement of the main ciliary trafficking machinery, intraflagellar transport (IFT), seems apparent. Vertebrate photoreceptor cells are ciliated sensory cells specialized for single photon detection. The photoreceptor outer segment corresponds to the ciliary shaft of a prototypic cilium.
