Benefit of Targeting a LDL (Low-Density Lipoprotein) Cholesterol \textless70 mg/dL During 5 Years After Ischemic Stroke
2020
Background and Purpose— The TST trial (Treat Stroke to Target) evaluated the benefit of targeting a LDL (low-density lipoprotein) cholesterol of <70 mg/dL to reduce the risk of cardiovascular events in 2860 patients with ischemic stroke with atherosclerotic stenosis of cerebral vasculature or aortic arch plaque >4 mm, in a French and Korean population. The follow-up lasted a median of 5.3 years in French patients (similar to the median follow-up time in the SPARCL trial [Stroke Prevention by Aggressive Reduction in Cholesterol Level]) and 2.0 years in Korean patients. Exposure duration to statin is a well-known driver for cardiovascular risk reduction. We report here the TST results …
Psychological support and psychotherapy via digital devices in Covid-19 emergency time: Some critical issues
2020
The Covid-19 pandemic and the resulting fear, quarantine and lockdown measures implemented in Italy and other countries to contain the risk of contagion have seriously impacted the mental health of a large number of people. The need to offer psychological and psychotherapeutic support to these people, while respecting the government’s pressing calls to “stay home”, have led many psychologists and psychotherapists, both in the public and private sectors, to provide their professional services via teleconference, telephone, smartphone, etc. The aim of this work is to highlight some critical issues related to the sudden switch from the traditional method of providing psychological services to…

«PER NON DAR MOTO ALLE FABBRICHE»: THE ROYAL ENGINEER SCIPIONE BASTA AND THE CONSTRUCTION OF THE ROANO CHAPEL IN THE CATHEDRAL OF MONREALE. In December 1686 the Royal Engineer Scipione Basta, a personality still largely to be investigated but which seems to portend a specific expertise in techno-constructive issues and in particular in structural reinforcement, was appointed by the Viceroyal Government to draw up a technical report concerning some delicate constructive aspects of the project of the architect fra Giovanni da Monreale for the chapel commissioned by the archbishop Roano for the cathedral. Due to the particular configuration of the building to be constructed –a domed octagonal …

Results The survey was answered by 34 (65%) out of 52 centers. NICU bedside surgery is reported in 81.8% of the pediatric surgery centers. A lower prevalence of bedside surgical practice in the NICU was reported for Southern Italy and the islands …
Two-Stage Thyroidectomy Driven by Intraoperative Neuromonitoring: Informed Consent Process and Its Effect on Patient Willingness and Consent Rates
2020
Purpose: Intraoperative neuromonitoring (IONM) of the recurrent laryngeal nerve (RLN) is a useful technique that can be applied to assess the nerve functionality at the end of the first side lobectomy in a planned total resection to prevent the bilateral injury of the RLN. Here we describe the process of informed consent of patients, who were subjected to a 2-stage thyroidectomy, and its effect on the patients' willingness to be operated on as well as their consent rates. Methods: A retrospective observational study of patients, undergoing thyroidectomy with standardized IONM, was conducted from January 2019 to December 2019. All patients were preoperatively informed about the possibility o…

Timeout procedure in paediatric surgery: effective tool or lip service? A randomised prospective observational study.
2021
BackgroundFor over a decade, the preoperative timeout procedure has been implemented in most paediatric surgery units. However, the impact of this intervention has not been systematically studied. This study evaluates whether purposefully introduced errors during the timeout routine are detected and reported by the operating team members.MethodsAfter ethics board approval and informed consent, deliberate errors were randomly and clandestinely introduced into the timeout routine for elective surgical procedures by a paediatric surgery attending. Errors were randomly selected among wrong name, site, side, allergy, intervention, birthdate and gender items. The main outcome measure was how freq…
P03.04 Signaling questions assessing brain tumor patients’ distress in clinical routine - a feasibility study
2019
Abstract BACKGROUND Approximately 20%-35% of patients with intracranial tumors show depressive symptoms and distress. Assessment in these patients remains challenging due to cognitive and/or neurological deficits. We developed 3 signaling questions in order to assess patients during patient-doctor consultation. The aim is to implement them in clinical routine and to compare the results with patient reported outcome measures (PROMs) along disease trajectory. MATERIAL AND METHODS Patients were prospectively examined in a structured interview applying the 3 following questions: 1),Has your mood worsened? (I)”; 2),Are you strained by physical changes? (II)”; 3),Has your faculty of thought decre…
[Informed consent in thyroid and parathyroid surgery].
2005
The Authors outline the law problems and the practice implications of thyroid and parathyroid surgery from the point of view of the informed consensus of the patient, and expose the criteria they usually adopt in their clinical practice for such procedures. This problem is particularly important because is exceptionally needed in emergency; therefore the preoperative information must be completed as possible, illustrating the risks of the procedure and the possible solutions alternative to surgery.
