Clinical management of drug-drug interactions in HCV therapy: Challenges and solutions.

2013

Contains fulltext : 118153.pdf (Publisher’s version ) (Open Access) Hepatitis C virus (HCV) infected patients often take multiple co-medications to treat adverse events related to HCV therapy, or to manage other co-morbidities. Drug-drug interactions associated with this polypharmacy are relatively new to the field of HCV pharmacotherapy. With the advent of the direct-acting antivirals telaprevir and boceprevir, which are both substrates and inhibitors of the cytochrome P450 (CYP) 3A iso-enzyme, knowledge and awareness of drug-drug interactions have become a cornerstone in the evaluation of patients starting and continuing HCV combination therapy. In our opinion, an overview of conducted dr…

A concise review of opioid-induced esophageal dysfunction: is this a new clinical entity?

2017

Opioids have become the most widely prescribed analgesics in Western countries. Opioid-induced bowel dysfunction is a widely known adverse effect, with constipation the most common manifestation. Most of the opioid-related effects occur in the stomach, small intestine, and colon and have been widely studied. However, the effects related to esophageal motility are less known. Recently published retrospective studies have suggested that long-term use of opioids can cause esophageal motility dysfunction, reflecting symptoms similar to motility disorders, such as achalasia and functional esophagogastric junction outflow obstruction. The most common manometric findings, as reported in the litera…

Variations in acoustical beam properties of intracoronary Doppler catheters.

1993

The limitations of coronary angiography in assessing the functional significance of coronary obstructions is well known. While the critical variable of coronary blood flow cannot be readily measured, intraluminal Doppler sonography offers useful related functional information on blood flow velocity. In order to fully evaluate Doppler signals it is essential to have exact knowledge of the transducer transmission characteristics and of the ultrasound beam topology. In an experimental set-up, the transmitter-receiver characteristics of five commonly used Doppler catheters were investigated. In comparing the beam characteristics we found inhomogeneities in the lateral beam spread. At a penetrat…

COVID-19 Vaccine: A Survey of Hesitancy in Patients with Celiac Disease

2021

(1) Background: COVID-19 vaccination campaigns offer the best hope of controlling the pandemic. However, the fast production of COVID-19 vaccines has caused concern among the general public regarding their safety and efficacy. In particular, patients with chronic illnesses, such as celiac disease (CD), may be more fearful. Information on vaccine hesitancy plays a pivotal role in the development of an efficient vaccination campaign. In our study, we aimed to evaluate COVID-19 vaccine hesitancy among Italian CD patients. (2) Methods: an anonymous questionnaire was sent to CD patients followed at our tertiary referral center for CD in Milan, Italy. Patients were defined as willing, hesitant an…

Mortality associated with cardiovascular disease in patients with COVID-19

2020

A Systematic Review of Cost-effectiveness Evidence of Endoscopic Saphenous Vein Harvesting: Is it Efficient?

2011

Objective: Greater saphenous vein harvest for coronary and lower extremity bypass requires the longest incision of any surgical procedure. Endoscopic vein harvest allows better results in some clinical variables compared to open harvesting techniques. The objec- tive of this study is to present the results of a systematic review of the scientific evidence about the efficiency of endoscopic saphenous vein harvest. Methods: We performed a systematic review in the bibliographical databases Pubmed, National Health Service Economic Evaluation Database, and NHS Health Technology Assess- ment Database. The search strategy was "endoscopic AND harvesting", in the period January 1970eDecember 2009. R…

Optimization of hepatitis C virus screening strategies by birth cohort in Italy

2020

Abstract Background and Aims Cost‐effective screening strategies are needed to make hepatitis C virus (HCV) elimination a reality. We determined if birth cohort screening is cost‐effective in Italy. Methods A model was developed to quantify screening and healthcare costs associated with HCV. The model‐estimated prevalence of undiagnosed HCV was used to calculate the antibody screens needed annually, with a €25 000 cost‐effectiveness threshold. Outcomes were assessed under the status quo and a scenario that met the World Health Organization's targets for elimination of HCV. The elimination scenario was assessed under five screening strategies. Results A graduated birth cohort screening strat…
