Long-term performance of the bovine pericardium patch in conventional carotid endarterectomy.
2014
Objective The aim of the study was to analyze long-term results of carotid endarterectomy (CEA) using bovine pericardium patch. Patients and Methods This study is a retrospective analysis of 274 consecutive cases (173 in CEA group and 101 patients in an internal control group of eversion endarterectomy [EEA]) operated between January 2005 and May 2007. Operations were performed according to standard surgical technique. Primary endpoints of the study were 30-day mortality, ipsilateral neurologic event rate, and high-grade restenosis in the long-term follow-up. Results No statistically significance between both groups was found in terms of gender, age, risk factors, medication (statine, plate…
Pulmonary endarterectomy in chronic thromboembolic pulmonary hypertension.
2018
Management and outcome of patients with operable chronic thromboembolic pulmonary hypertension (CTEPH) who underwent pulmonary endarterectomy (PEA) at a large German referral center were investigated. In Germany, 394 PEAs were performed in 2014 and 2015 with an in-hospital mortality rate of 5.8%. Of these, 253 patients (64.2%) were treated at the Kerckhoff Clinic, Bad Nauheim, and 237 (93.7%; median age, 62 years [interquartile range [IQR], 52-72 years]; 46.0% female) were included in the present analysis. On referral, 52 patients (22.0%) were treated with pulmonary arterial hypertension-specific drugs and 95 (40.4%) were treated with non-vitamin K-dependent oral anticoagulants, and 14 (5.9%)…

Severity of Asymptomatic Carotid Stenosis and Risk of Ipsilateral Hemispheric Ischaemic Events: Results from the ACSRS Study
2005
Objectives. This study determines the risk of ipsilateral ischaemic neurological events in relation to the degree of asymptomatic carotid stenosis and other risk factors. Methods. Patients (n = 1115) with asymptomatic internal carotid artery (ICA) stenosis greater than 50% in relation to the bulb diameter were followed up for a period of 6-84 (mean 37.1) months. Stenosis was graded using duplex, and clinical and biochemical risk factors were recorded. Results. The relationship between ICA stenosis and event rate is linear when stenosis is expressed by the ECST method, but S-shaped if expressed by the NASCET method. In addition to the ECST grade of stenosis (RR 1.6; 95% CI 1.21-2.15), histor…
carotid endarterectomy versus stenting in patients with contralateral carotid artery occlusion

General anaesthesia versus local anaesthesia for carotidsurgery (GALA): a multicentre, randomised controlled trial.
2008
Background The eff ect of carotid endarterectomy in lowering the risk of stroke ipsilateral to severe atherosclerotic carotid-artery stenosis is off set by complications during or soon after surgery. We compared surgery under general anaesthesia with that under local anaesthesia because prediction and avoidance of perioperative strokes might be easier under local anaesthesia than under general anaesthesia. Methods We undertook a parallel group, multicentre, randomised controlled trial of 3526 patients with symptomatic or asymptomatic carotid stenosis from 95 centres in 24 countries. Participants were randomly assigned to surgery under general (n=1753) or local (n=1773) anaesthesia between J…
Proximal vertebral artery surgery. Results of treatment of ischemia of the rhombencephalon (hindbrain). Simultaneous carotid and vertebral artery dec…
2022
Surgical treatment of hindbrain ischemia is becoming the accepted surgical procedure in cases of stenosis of extracranial segments of vertebral arteries in the course of advanced atherosclerotic lesions in their initial segments (V1) or caused by external compression of these arteries in the canal formed by the transverse processes of cervical vertebrae. The latter is referred to as vertebrobasilar insufficiency (VBI). The transposition of the left vertebral artery to the common carotid artery in cases where stent graft implantation is necessary with coverage of the subclavian artery and possible blood supply disorders of both the hindbrain and the spinal cord has become important. In the D…
Operācijas rezultātu korelācija ar miega artērijas revaskularizācijas metodēm
2014
Anotācija Miega artēriju stenozes ķirurģiskai korekcijai karotīdu endarterectomijai ir būtiska nozīme insulta profilaksē. Promocijas darba mērķis: Izvērtēt miega artērijas endarterektomiju operāciju rezultātus, ņemot vērā intima-media slāņa biezumu un pacienta dzīves kvalitāti. Promocijas darbā autore veikusi pētījumu, kas atspoguļo karotīdu endarterektomijas operāciju rezultātus (komplikācijas, dzīves kvalitātes izmaiņas) un intīmas hiperplāzijas attīstību postoperatīvā periodā. Izveidota speciāla dzīves kvalitātes novērtēšanas anketa pacientiem ar miega artērijas sašaurinājumu. Pētījuma materiālu veido 104 pacienti, kuriem veiktas karotīdu endarterektomijas. Darbs veikts Latvijas Universi…
Interaction of cerebrovascular disease and contralateral carotid occlusion in prediction of shunt insertion during carotid endarterectomy
2012
INTRODUCTION To assess the possible role and the interaction of cerebrovascular disease and vascular stenosis on the necessity of shunt insertion during carotid endarterectomy (CEA). MATERIAL AND METHODS Eighty consecutive patients undergoing CEA under regional anaesthesia were prospectively enrolled. Patients were divided into two groups depending on whether they were shunted or not. The measured end-points were co-morbidities degree of contralateral and carotid stenosis and other intra- and postoperative outstanding parameters. ANOVA, Student's t and χ(2) tests were used (p<0.05). Variables differing significantly between groups and potential confounders were used in backward stepwise log…
