PROBLEM The anatomically shaped pcl (ESKA) total hip replacement has a porous coating of the proximal stem and the cup to allow for bony ingrowth. The tip of the stem is polished. METHOD We report the results of 96 THR 6.2 years (5.25-6.7) after implantation. RESULTS There were no revisions during this period. One stem showed radiographic loosening at follow-up. There was stress-shielding in 9.4% in contrast to the concept of proximal fixation. Thigh-pain was found in 9.4%. One patient demonstrated disabling thigh-pain despite radiologic bony ingrowth. Is there still an indication for operative treatment of femoral neck fractures with a ceramic hemiprosthesis?

From 1985 to 1995, 417 patients with dislocated medial femoral neck fractures (Garden III–IV) were treated with hemiarthroplasty using a Biolox ceramic head. The average patient age at the time of operation was 81.5 years. A total of 140 survivors was available for follow-up examination with a mean prosthesis longevity of 55.8 months. The Harris hip score recorded a mean of 70.6 points. At the time of follow up, 5 patients had severe hip pain, and in 8 the roentgenographic examination revealed protrusio acetabuli. Five of these 8 patients underwent revision surgery for replacement of the cup, leaving the stem in situ.

CHF2819 is a novel orally active acetylcholinesterase inhibitor (AChEI) developed for the treatment of Alzheimer's disease (AD). CHF2819 is a selective inhibitor of AChE, it is 115 times more potent against this enzyme than against butyrylcholinesterase (BuChE). Moreover, CHF2819 is more selective for inhibition of central (brain) AChE than peripheral (heart) AChE. In vivo CHF2819, 0.5, 1.5, and 4.5 mg/kg p.o., significantly and in dose-dependent manner increased acetylcholine (ACh) levels in hippocampus of young adult rats. OBJECTIVE: After hip fracture, muscle strength and power remain persistently poor, especially in the fractured leg. This study explores whether asymmetrical leg extension power (LEP) deficit affects mobility in women after proximal femoral fracture (PFF). DESIGN: In this observational study, LEP of both legs, 10- and 50-foot walking speed, and stair-climbing speed were measured in 43 women, aged 73-96, at 1 and 13 wks after surgical repair of PFF. Asymmetrical LEP deficit was calculated as (fractured/(sum both legs)) x 100%.
