Paving the way of systems biology and precision medicine in allergic diseases: the MeDALL success story: Mechanisms of the Development of ALLergy; EU…
2016
MeDALL (Mechanisms of the Development of ALLergy; EU FP7-CP-IP; Project No: 261357; 2010-2015) has proposed an innovative approach to develop early indicators for the prediction, diagnosis, prevention and targets for therapy. MeDALL has linked epidemiological, clinical and basic research using a stepwise, large-scale and integrative approach: MeDALL data of precisely phenotyped children followed in 14 birth cohorts spread across Europe were combined with systems biology (omics, IgE measurement using microarrays) and environmental data. Multimorbidity in the same child is more common than expected by chance alone, suggesting that these diseases share causal mechanisms irrespective of IgE sen…

Is it possible to make a diagnosis of raw, heated and baked egg allergy in children using cut-offs? A systematic review
2015
The diagnosis of IgE-mediated egg allergy lies both on a compatible clinical history and on the results of skin prick tests (SPTs) and IgEs levels. Both tests have good sensitivity but low specificity. For this reason, oral food challenge (OFC) is the ultimate gold standard for the diagnosis. The aim of this study was to systematically review the literature in order to identify, analyze, and synthesize the predictive value of SPT and specific IgEs both to egg white and to main egg allergens and to review the cutoffs suggested in the literature. A total of 37 articles were included in this systematic review. Studies were grouped according to the degree of cooking of the egg used for OFC, age…
Beyond good and evil: The impact of digital environment in family life
2019
Internet has changed the way we interact with others and has become an essential tool for daily activities. Children and adolescents are the main users of new digital technologies and their first contact with a screen happens increasingly earlier in life. Its premature use can be dangerous: technology represents a “supernormal stimulus” that can act on the brain reward circuit and affect neural development. In this article we describe the effects of technology on the brain, well-being, social and family relationships, highlighting both positive and negative ones: rather than defining a priori any type of technology as “good” or “bad”, we should consider how specific applications are used in…
Fractional exhaled nitric oxide is associated with more severe asthma
2018
Fractional exhaled nitric oxide (FeNO) levels are used as a marker of airway inflammation. The aim of the present work was to evaluate a possible relationship between FeNO and parameters of asthma control and severity in different asthma phenotypes. FeNO was evaluated in 200 asthma patients (37% male, mean age ± SD 54±15 years, 19% controlled (GINA), 30% treated with oral corticosteroids (OCS), median FEV1 (interquartile range) 2,05 L (1,49-2,70), 74% pred. (56-90%)), together with other asthma characteristics (lung function, asthma control, allergies, serum IgE, serum ECP, and blood eosinophils). Patients with poorly controlled asthma had significantly higher FeNO values (ACQ-5≥1.5 vs. 70%…
Long-term efficacy of omalizumab (OMA) for patients with severe allergic asthma (SAA). Clinical assessment and relationship to serum IgE concentratio…
2015
Background: OMA is an effective treatment option in SAA. However, little data are available on the long-term-efficacy oft the drug. Objective: To determine the long-term efficacy of OMA in respect to clinical parameters and IgE serum levels. Methods: This was a retrospective, open-label, uncontrolled, multi-center observational study according to the definition of non-interventional studies. 22 non-smoking patients (10 m, 12 f; mean age 47,9 yrs) with SAA were enrolled. Pat were treated with OMA up to for 4 years and total serum IgE levels were assessed at initiation of therapy, after 2-6 ms and 3-5 yrs. Data were recorded on standardized CRFs by the treating physician. Results: The average…
Immunoglobulin E (IgE) and ischemic heart disease. Which came first, the chicken or the egg?
2014
Several lines of evidence demonstrate that the immune system plays a pivotal role in development and progression of ischemic heart disease (IHD). More recently, a series of biological and clinical investigations has generated new interest about the existence of a relationship between a specific class of immunoglobulin, that is immunoglobulin E (IgE), and IHD. Data obtained in several epidemiological studies have convincingly demonstrated that the concentration of total serum IgEs is significantly increased in patients with IHD and often correlates with the prognosis. The putative mechanisms are essentially mediated by a physiological interaction between IgEs and mast cells, which triggers t…
