Papillary thyroid microcarcinoma: extrathyroidal extension, lymph node metastases and risk factors for recurrence in a high prevalence of goiter area

2010

BACKGROUND. Papillary thyroid microcarcinoma (PTMC) is diagnosed with increasing frequency. Since risk factors for an aggressive clinical behavior have not been clearly delineated, the best therapeutic option and follow up protocol for PTMC have not been established yet, especially in case of incidental diagnosis after partial thyroidectomy for benign disease. We reviewed our series of patients with PMTC to determine risk factors for recurrence in a high prevalence of goiter area. METHODS. The medical records of all the patients who underwent total thyroidectomy (TT) with a final pathology report of PTMC between October 2002 and June 2007 were reviewed. Demographic, clinical and pathologica…

OR18-03 Functional TSH Receptor Antibodies Are a Biomarker for Graves’ Disease - a Prospective Trial

2020

Abstract Objective We aimed to evaluate the clinical utility and predictive value of stimulatory (TSAb) and blocking (TBAb) TSH receptor antibodies in the management of Graves’ disease (GD). Methods Hundred well-defined, consecutive, unselected, untreated hyperthyroid patients with GD were enrolled in a prospective two-year trial. Methimazole (MMI) was administered for 24 weeks according to baseline serum concentrations of free T3/free T4. Starting dose was 5–30 mg/day. Through a titration regimen, this dose was respectively tapered or increased at each subsequent study visit as the patient became euthyroid or remained hyperthyroid. Goals of therapy were to maintain normal fT4 and TSH level…

Prognostic impact of extra-nodal extension in thyroid cancer: A meta-analysis

2015

Background and Objectives Lymph node involvement is common in thyroid cancer, but the system of staging does not consider the histological features of lymph node metastases. We conducted a meta-analysis to investigate the prognostic role of extranodal extension (ENE) in thyroid cancer patients. Methods We ran PubMed and SCOPUS searches without language restrictions. Prospective studies reporting data on overall mortality, cancer-specific mortality, or disease recurrence including thyroid cancer patients, in which cases with ENE (ENE+) were compared with those with only intranodal disease (ENE-) were eligible. Data were summarized using risk ratios (RR) for number of deaths/recurrences, and …

Human Thyroid Cancer Stem Cells

2011

Increasing advances in stem cell research have opened new chances for treatment of many types of cancers. The identification of Cancer Stem Cells (CSCs) will provide a better knowledge of the molecular events governing carcinogenesis and the establishment of more accurate diagnostic methods and effective therapies. Parallel to other tumors, it has been demonstrated the presence of CSCs in thyroid cancers. Here, we will discuss the origin of human thyroid cancer stem cells and what is known about the molecular and the cellular aspects of their biology. We will also explore the potential traits of CSCs in thyroid cancer which give hope for the development of new therapeutic approaches in trea…

The New TNM Staging System for Thyroid Cancer and the Risk of Disease Downstaging

2018

In October 2016 the American Joint Committee on Cancer (AJCC) published the 8th edition of the AJCC/TNM cancer staging system and it has been introduced in clinical practice since 1st January 2018. The effect of most of the changes in the new edition was the downstaging of a significant number of patients into lower stages, reflecting their low risk of thyroid cancer-related death. One of the most relevant modification refers to the role of the microscopic extra-thyroidal tumor invasion, which is no longer considered as criterion for the classification of T3 tumors. With the present study we want to assess the impact of the changes of the new staging system and therefore we analyzed or casi…

[Informed consent in thyroid and parathyroid surgery].

2005

The Authors outline the law problems and the practice implications of thyroid and parathyroid surgery from the point of view of the informed consensus of the patient, and expose the criteria they usually adopt in their clinical practice for such procedures. This problem is particularly important because is exceptionally needed in emergency; therefore the preoperative information must be completed as possible, illustrating the risks of the procedure and the possible solutions alternative to surgery.

Pili Annulati Coincident with Alopecia Areata, Autoimmune Thyroid Disease, and Primary IgA Deficiency: Case Report and Considerations on the Literatu…

2012

Pili annulati is a rare autosomal dominant hair disorder clinically characterized by a pattern of alternating bright and dark bands of the hair, the bright bands appearing dark if observed by transmitted light. This pattern is due to the periodic occurrence of air-filled cavities along the hair cortex which scatter and reflect the light while precluding its transmission. A susceptibility region, including a possibly responsible Frizzled gene, has been mapped to the telomeric region of chromosome 12q, although a specific mutation has not been identified. The condition has sometimes been observed in concurrence with alopecia areata, and in this paper we report a case in whom the concomitant s…

Anaplastic Thyroid Cancer in Sicily: The Role of Environmental Characteristics

2017

Background: Anaplastic thyroid cancer (ATC) is a rare but extremely aggressive cancer of the thyroid, contributing up to 30-40% of thyroid cancer-specific mortality. We analyzed ATC characteristics and survival rates in Sicily to evaluate the possible influence of environmental factors. With this aim data regarding ATC incidences in urban/rural and industrial, iodine deficient and volcanic vs. control areas were compared in Sicily as well as ATC data from Sicily and USA. Methods: Using the Sicilian Register of Thyroid Cancer (SRTC) database incidence, age, gender, tumor size and histotype, extrathyroidal extension, stage and coexistence with pre-existing differentiated thyroid cancer (DTC) …
