Behaviour of the plasma concentration of gelatinases and their tissue inhibitors in subjects with venous leg ulcers.

Venous leg ulcers are common in subjects with chronic venous insufficiency. The increased intraluminal pressure causes alteration of the skin microcirculation, leukocyte activation and release of proteolytic enzymes leading to ulceration. An impaired expression and activity of matrix metalloproteases (MMPs) and their tissue inhibitors (TIMPs) might influence extracellular matrix degradation and deposition in chronic venous ulcers with the failure of the healing process. Correction of the unfavourable effects of vasopressin by nitroglycerin infusion

Nitroglycerin was administered with vasopressin to prevent adverse effects. Vasopressin 0.25U . 70 kg-1 min-1 was infused intravenously in four dogs for 40 minutes, when a venous infusion of nitroglycerin 1.2 micrograms . kg-1 . min-1 was added for 20 minutes. Nitroglycerin 1.2 micrograms . kg-1 . min-1 alone was infused intravenously in another four dogs for 40 minutes. The venous blood pressures (mesenteric and central) and arterial pressures (mesenteric and femoral), the electrocardiogram and arterio-venous difference were recorded. Nitroglycerin was shown to annul the unfavourable effects of vasopressin without altering its efficacy upon portal pressure.

Chronic venous disease (CVD) is an often underestimated inflammatory pathological condition that can have a serious impact on quality of life. Many therapies have been proposed to deal with CVD, but unfortunately the symptoms recur with increasing frequency and intensity as soon as treatments are stopped. Obstructive malformations of the internal jugular veins represent a relatively new clinical problem, which as of yet has not been fully understood. In normal subjects, the internal jugular vein is valveless, except for a single valve, situated just above the junction of this vein with the brachiocephalic vein.
