Sudden severe abdominal pain after a single low dose of paracetamol/codein in a cholecystectomized patient: learning from a case report.

We report the case of an elderly patient with diastolic heart failure and renal insufficiency admitted to hospital as he complained of having a history of hypogastric pain and dysuria without fever due to renal lithiasis and urinary infection. Because the pain was persistence, and considering the presence of renal dysfunction, it was administered a single low dose of paracetamol/codein (500/30 mg). After about 1 hour of the administration, he suddenly complained of the onset of a lancinating epigastric pain radiating to the whole abdomen and retrosternum accompanied by nausea. Paracetamols jau vairāk kā simts gadus ir vienas no pasaulē plašāk lietotajām pretsāpju un pretdrudža zālēm. Paracetamols ir pierādīts kā drošs medikaments, lietojot atbilstošas un kontrolētas devas, tomēr visbiežāk ziņotā paracetamola blakusparādība ir hepatotoksicitāte, kas rodas pēc akūtas pārdozēšanas. Darba mērķis bija noskaidrot, kurus paracetamolu saturošus bezrecepšu medikamentus pircēji izvēlas un šo medikamentu apriti “Galma aptiekā” un “Ozolnieku aptiekā” laika periodā no 2019. gada 1. janvara līdz 2020. gada 31. decembrim. Introduction: Idiosyncratic drug-induced liver injury (DILI) is a rare adverse reaction to drugs and other xenobiotics. DILI has different grades of severity and may lead to acute liver failure (ALF), for which there is no effective therapy. N-acetylcysteine (NAC) has been occasionally tested for the treatment of non-acetaminophen drug-induced ALF. However, limited evidence for its efficacy and safety is currently available.
