Research and Clinical Routine in the next Century,Segregation or Cooperative Networks?
2001
Reading the daily newspaper one gets the impression that German universities and clinical research in particular are in a deep crisis, and that German professors are to be blamed for it. In medicine, additional insufficiencies of the system are obvious, beginning with corruption cases brought up against clinicians and ending with conflicts of interest due to the fact that clinical professors in addition to research and teaching devote much of their time to the special care of private patients. These headlines in the news tend to let us forget that during the last decade grant moneys for research projects stagnated whereas the number of grant applications has increased out of proportion. In …
Asymptomatic ventricular pre-excitation in children.
2009
ASYMPTOMATIC VENTRICULAR PREEXCITATION IN CHILDREN OBJECTIVE: This retrospective study was planned for a good risk assessment of asymptomatic patients affected by ventricular pre-excitation. METHODS: From 1985 to 2007, 124 patients with an atrioventricular pathway (electrocardiographic signs of ventricular pre-excitation) were admitted to our cardiology division. The average age was 7 years (range 1 month to 18 years). The mean follow-up period in the whole population of patients was 4.2 years (range 1-13 years). Four patients were lost during the follow-up. During this period, all patients remained in good health. In all of them, we performed a Holter evaluation every year. An intermittent…
Risk assessment in ginecology and obstetrics in Sicily: an approach based on Wolff's Criteria
2013
Objectives To apply Wolff’s Criteria to hospital discharge records (HDR) in order to detect adverse events worthy of further study. Methods Gynecology and Obstetrics Units of three Sicilian hospitals were considered and HDR regarding ordinary and day hospital admissions in 2008 were collected. A matched case-control study was designed, by random selection of 10 controls at maximum for each case. Matching was performed on the variables age and speciality of admission (gynecology or obstetrics). Results Out of a total of 7011 HDR examined, 114 cases were identified with Wolff’s Criteria. Multivariate analysis confirmed a statistically significant association with the origin of admission, diag…
Inanspruchnahme von Selbsthilfegruppen und Psychotherapie im Anschluss an eine stationäre psychosomatisch-psychotherapeutische Behandlung
2007
Until now little is known about the role of participation in self-help groups alone or combined with psychotherapy in post-in-patient care. In the present study 2933 patients were questioned about their experience of self-help groups and psychotherapy after discharge from a clinic for psychosomatic medicine and psychotherapy. Nearly 8 % of them utilized self-help groups (mostly combined with out-patient psychotherapy), and altogether 68 % out-patient psychotherapy following in-patient treatment. Patients without out-patient treatment were psychologically less burdened and had better resources than participants of self-help groups or psychotherapy. Self-help group members differed from patie…
Extemporaneous formulations in Germany – relevance for everyday clinical practice
2017
Background Extemporaneous formulations broaden the spectrum of therapeutic options for topical treatment in particular and thus improve patient care. The latest amendment to the Regulation on the Operation of Pharmacies issued in 2012 brought about changes in prescribing and manufacturing practices. The aim of the present study was to assess the relevance of extemporaneous formulations in everyday clinical practice. Methods We used data from the German Institute for Drug Use Evaluation (DAPI) to analyze the prescribing practice for compounded preparations in Germany between the fourth quarter of 2011 and the third quarter of 2014. In doing so, we determined the total cost associated with ex…

Monitorización de la vacomicina en administración intraperitoneal en pacientes sometidos a diálisis peritoneal continua ambulatoria
2012
Objective: To validate a pharmacokinetic model of the treatments with intraperitoneal vancomycin applied to patients on continuous ambulatory peritoneal dialysis with bacterial peritonitis. Methods: To carry out a prospective study divided in 2 cohorts: the first one including ten patients of 56 ± 14 years and 65 ± 5 kg, and the second one with 10 patients (12 episodes of peritonitis) aged 52 ± 13 years and 64 ± 8 kg. The treatment consists of administering and retaining for 6 h in the peritoneal cavity a solution containing 2 g of vancomycin and 1 g of ceftazidime into 2 l of ‘‘dialysis solution’’. After the antibiotic administration, blood samples were obtained at 4, 6, 8, 10, 24, 48 and …
Is Complexity of Daily Activity Associated with Physical Function and Life Space Mobility among Older Adults?
2022
Purpose Information about mobility, and physical function may be encoded in the complexity of daily activity pattern. Therefore, daily activity pattern complexity metrics could provide novel insight regarding the relationship between daily activity behaviour and health. The purpose of the present study was to examine the association between the complexity of daily activity behaviour, and mobility and physical function among community-dwelling older adults aged 75, 80, and 85 years-of-age. Methods A total of 309 participants wore accelerometers concurrently on the thigh and the trunk for at least 3 consecutive days. Five activity states (lying, sitting, standing, walking, or activity other t…
Methodology and technology for peripheral and central blood pressure and blood pressure variability measurement: current status and future directions…
2016
Office blood pressure measurement has been the basis for hypertension evaluation for almost a century. However, the evaluation of blood pressure out of the office using ambulatory or self-home monitoring is now strongly recommended for the accurate diagnosis in many, if not all, cases with suspected hypertension. Moreover, there is evidence that the variability of blood pressure might offer prognostic information that is independent of the average blood pressure level. Recently, advancement in technology has provided noninvasive evaluation of central (aortic) blood pressure, which might have attributes that are additive to the conventional brachial blood pressure measurement. This position …
Mild obstructive sleep apnoea: clinical relevance and approaches to management
2015
Summary Obstructive sleep apnoea is highly prevalent in the general population worldwide, especially in its mild form. Clinical manifestations correlate poorly with disease severity measured by the apnoea–hypopnoea index (AHI), which complicates diagnosis. Full polysomnography might be more appropriate to assess suspected mild cases because limited ambulatory diagnostic systems are least accurate in mild disease. Treatment options in mild obstructive sleep apnoea include continuous positive airway pressure (CPAP) and oral appliance therapy, in addition to positional therapy and weight reduction when appropriate. The superior efficacy of CPAP in reducing AHI is offset by greater tolerance of…
