Miliary tuberculosis infection during hepatitis C treatment with sofosbuvir and ledipasvir plus ribavirin

Chronic hepatitis C virus (HCV) infection is one of the main causes of chronic liver disease worldwide. In the last 5 years, treatment for HCV infection has experienced a marked development. In 2014, the use of ledipasvir/sofosbuvir with or without concomitant weight-based ribavirin was approved with a very significant increase in the sustained virological response. However, new side effects have been associated. We report the first case of an HCV infected patient treated for 12 wk with the combination of sofosbuvir/ledipasvir plus ribavirin who developed a miliary tuberculosis (TB) infection while on therapy. Carbon-rich layers exist at both sides of the Mediterranean Sea sedimentary record and are called sapropels and organic rich layers (ORLs), respectively in the eastern and western basins. They have different levels of organic carbon accumulation and seafloor oxygen deprivation. The most recent sapropel and ORL deposition have a different timing, 10.8-6.1 and 14.5-9.0 ka respectively. Here we investigate oxygen isotopic records of three foraminifera species that occupy different habitats within the Sicily Channel water column since ~ 12.0 ka, thus in the sill between the eastern and western Mediterranean basins. With clinical purposes in mind, a review of the proximal/distal opposition is carried out in order to define a universal parameter of variability in semiotic procedures. By taking into consideration different-although notionally inter-related-senses of the proximal/distal opposition, a cluster of semiotic properties is proposed, which initially permits one to characterize dimensions of variability in the form and use of gestures.
