Chronic atrial fibrillation in presence of aortic stenosis in a patient with polysplenia syndrome.
2013
We report a rare case of “situs viscerum ambiguous” with polysplenia syndrome, in a 69 year old female patient with aortic stenosis and chronic atrial fibrillation. The presenting symptom was dyspnoea on moderate exertion and an ECG showed supra ventricular arrhythmia. Patients trans-thoracic echocardiogram revealed a dilated left atrium, reduced ejection fraction, mild tricuspid regurgitation, moderate-severe pulmonary hypertension and severe aortic stenosis. The patient was successfully treated with a replacement of her aortic valve and ascending aorta.
Three-Dimensional Echocardiographic Evaluation of Aortic and Mitral Valve Stenosis
2001
Dynamic volume rendered three-dimensional echocardiography allows the spatial recognition of anatomy and function of the aortic and mitral valves with acceptable image quality. The aortic valve can be best visualized in a view from the ascending aorta down to the valve level, thus allowing an overview of the aortic aspect of the valve in a surgeon's perspective in ∼ 80% of patients. Planimetric measurement of the aortic valve area was possible in 88% of patients, and there is no systematic overestimation or underestimation of aortic valve area compared with two-dimensional echocardiography and catheterization. The entire valvular circumference of the mitral valve can be assessed from both a…
Isomeric arrangement of the left atrial appendages and visceral heterotaxy: two atypical cases.
2000
AbstractWe present two cases in which there was isomeric arrangement oi the left atrial appendages in association with multiple spleens and complete heart block. In both of these, the venous connections were normal. In one case the arrangement of the bronchial tree and the lungs was normal, the liver was left-sided, and the stomach was right-sided. In the other case, there was left isomeric broncho-pulmonary morphology, and both liver and stomach were right-sided. These unusual combinations show the need for full description of the morphology of other organs, and the venoatrial connections, in cases with isomeric arrangement of the atrial appendages. They also demonstrate that connection of…
Initial and long-term antithrombotic therapy after left atrial appendage closure with the WATCHMAN
2020
Abstract Background Evidence regarding post-procedural antithrombotic regimes other than used in randomized trials assessing percutaneous left atrial appendage (LAA) closure is limited. Purpose The present work aimed to compare different antithrombotic strategies applied in the real-world EWOLUTION study. Methods A total of 998 patients with successful WATCHMAN implantation at 47 centers were available for the present analysis. The composite ischemic endpoint of stroke, TIA, systemic embolism and device thrombus as well as the bleeding endpoint defined as at least major bleeding according to BARC were assessed during an initial period (from implant until first medication change) and long-te…
Morphology-based measurement of activation time in human atrial fibrillation
2003
The measurement of the activation time is crucial to allow the correct automatic analysis and classification of intracardiac electrograms recorded in the human atria during atrial fibrillation (AF). This study proposes a method which accounts for the morphology of bipolar signals. After ventricular artifact removal and activation wave recognition, the fiducial point of the activation wave was set at its local barycentre (LB). The method was tested on a set of 30 AF bipolar recordings of increasing complexity class; its performance was compared with that of the traditional methods of maximum peak (MP) or maximum slope (MS) estimation, taking the manual measurements performed by an expert car…
Carotid plaque detection improves the predictve value of CHA2DS2-VASc score in patients with non-valvular atrial fibrilation: The ARAPACIS Study
2017
Abstract Background and aims Vascular disease (VD), as assessed by history of myocardial infarction or peripheral artery disease or aortic plaque, increases stroke risk in atrial fibrillation (AF), and is a component of risk assessment using the CHA 2 DS 2 -VASc score. We investigated if systemic atherosclerosis as detected by ultrasound carotid plaque (CP) could improve the predictive value of the CHA 2 DS 2 -VASc score. Methods We analysed data from the ARAPACIS study, an observational study including 2027 Italian patients with non-valvular AF, in whom CP was detected using Doppler Ultrasonography. Results VD was reported in 351 (17.3%) patients while CP was detected in 16.6% patients. Ad…
Organization measures of atrial activity during fibrillation
2008
The present chapter summarizes the state-of-art of how to quantify the degree of atrial organization during AF through a review of the main signal processing techniques employed for the analysis of atrial electrical activity. As the concept of atrial organization may assume different meanings in the context of AF, particular attention is paid to stress those peculiar characters of organization probed and captured by each method.
The natriuretic system in human and murine submandibular salivary glands in health and disease: insights from head and neck tumour cases
2019
Introduction: The wide distribution of the atrial natriuretic peptide was paralleled by a similar presence of its receptors, an indication of the wide range of their biological effects beyond the control of the homeostatic process. This study depicts through immunohistochemical and molecular techniques the distribution and modulation of atrial natriuretic peptide and its receptors in human and murine salivary glands. It also highlights some clinical implications of ANP dysregulation. Methods: Human SMG’s tissue samples were biopsied from normal (3), people with benign (2) or with malignant tumors (8). In addition, biopsies were taken from normal mice (10). They were processed by standard me…

Can Ventricular Resynchronization Reduce Atrial Fibrillation Recurrences?
2003
In recent years a new pacing therapy has been proposed for patients affected by heart failure (HF) in order to reduce inter- [1,2], intra- [3,4], and atrioventricular [5] (AV) dyssynchrony. Cardiac resynchronization therapy (CRT) has the goal of correcting these hemodynamic disorders, thus improving left ventricular (LV) performance. The benefits of CRT have been evaluated in a series of clinical trials [6-10].
