Isolated Endometriosis on the Rectus Abdominis Muscle in Women without a History of Abdominal Surgery: a Rare and Intriguing Finding

We report 2 rare cases of endometriosis on the rectus abdominal muscle diagnosed incidentally during an operation for inguinal hernia repair in women with no surgical history. Two women sought medical attention for a mass found in the pubic abdominal wall. Only 1 woman reported occasional pain. At physical examination in both women, an ovoid swelling in the right pubic area was felt. One woman experienced pain on palpation, and one reported slight discomfort. Ultrasonography demonstrated a heterogeneous hypoechogenic formation with indistinct edges; diagnosis was difficult. Vaginal reconstruction with bilateral island "extended" groin flaps: description of a personal technique.

We propose a vaginal reconstruction method using two island "extended" groin flaps designed over the iliac crests and in axis with the superficial circumflex iliac arteries. After a surgical revision of the pelvic organs and dissection of the area between the bladder and the rectum, the two flaps pass, extraperitoneally by way of the abdomen, through an incision made along the lateral margin of the rectus muscle and, from before to behind, through the fibrovascular lamina lateral to the bladder, and finally are placed in the area between the bladder and the rectum, where they are sutured. The proper differentiation and maintenance of myofibers is fundamental to a functional musculature. Disruption of numerous mostly structural factors leads to perturbations of these processes. Among the limited number of known regulatory factors for these processes is Mind bomb2 (Mib2), a muscle-associated E3 ubiquitin ligase, which was previously established to be required for maintaining the integrity of larval muscles.
