A case report on dental management of a toddler with Pachygyria

Children with special health care needs receive less oral care than the normal population, inspite of the high level of dental diseases among them. They are at an increased risk for oral diseases throughout their lifetime. This paper reports a case of a toddler with congenital unusual thick convolutions of the cortex resulting in a condition called pachygyria. Intra oral examination showed multiple abscesses with poor oral hygiene. As the patient was lacking cooperative ability, treatment of full mouth rehabilitation as needed. In this work the colour magnification technique was applied for monitoring of palm skin microcirculation changes under peripheral (Plexus Brachialis with axiliary access) Regional Anaesthesia (RA). During the RA procedure 20 minute video of patient’s forearm was taken at steady light conditions. It was the aim of this study to identify all publications with a focus on paediatric anaesthesia, and to determine the spectrum of topics, as well as the publication type and language for the period between 1993 and 1998. All articles published in 12 major anaesthesia journals were evaluated and, additionally, a computerized, Internet-based Medline-search was performed using selected keywords. The occurrence of adverse reactions is decreasing since the introduction of non-ionic contrast media. Anaphylactoid reactions during general anaesthesia are rare and hitherto only documented with the administration of ionic compounds. We report an episode of hypotension, tachycardia, bronchospasm and urticaria following application of a non-ionic contrast medium (Iopamidol) during isoflurane anaesthesia. We conclude that a combinent use of anaesthesia and non-ionic contrast media does not guaranty protection from anaphylactoid reactions to iodinated radiopaque compounds.
