The role of endothelium-related factors in the pathogenesis of cerebral vasospasm after subarachnoid hemorrhage (SAH) has gained interest since the discovery of endothelin-1 (ET-1). We have examined, before and after SAH, the responsiveness of the cerebrovascular bed of the goat to ET-1, the sources of Ca2+ in ET-1-induced responses, and the ability of the Ca2+ entry blocker nicardipine to counteract them. A common component in many protocols for the evaluation of cerebral autoregulation is the comparison of transcranial Doppler ultrasound (TCD) velocities with blood pressure recordings, in which correlations between these two signals correspond to impaired autoregulation. Subarachnoid hemorrhage (SAH) following aneurysm bleeding accounts for 6% to 8% of all cerebrovascular accidents. Although an aneurysm can be effectively managed by surgery or endovascular therapy, delayed cerebral ischemia is diagnosed in a high percentage of patients resulting in significant morbidity and mortality. Mucoid impaction and plastic bronchitis are relatively rare disorders affecting patients of any age. Here we report the case of mucoid impaction developing in a four-year-old child as a complication of pneumonic infection treated four weeks prior. p.a. chest X-ray and fiberoptic bronchoscopy showed atelectasis and the location of a mucus cast in the left upper lobe. After extraction of the cast via the suction channel of the bronchoscope had failed, we administered 2.5 mg of undiluted recombinant human deoxyribonuclease (rhDNase) instilled directly over the affected area of occlusion for 15 minutes. This mucolytic agent mobilized the cast by reducing viscoelasticity and adherence of mucus. …

Cardiopunkture in anesthesia is a rapid and accurate method for withdrawal of blood and injection without as many complications as suspected if carried out correctly. In the animal placed on its back a syringe with a gauge-24 needle is inserted in the left 4th or 5th intercostal space close to the sternum. When the needle is slowly pushed forward toward the spine continuous suction is exerted with the plunger. After blood has been drawn the needle is removed with a quick jerk. Thus when rats were punctured 8 to 18 times only 1 animal died of a hemopericard in 275 punctures. No other complication could be observed.

For the evaluation of nasal airway obstruction physical examination, anterior rhinoscopy, laboratory workup, imaging studies, and rhinomanometric studies may be required.
