CMC is more than a measure of corticospinal tract integrity in acute stroke patients

2021

In healthy subjects, motor cortex activity and electromyographic (EMG) signals from contracting contralateral muscle show coherence in the beta (15-30 Hz) range. Corticomuscular coherence (CMC) is considered a sign of functional coupling between muscle and brain. Based on prior studies, CMC is altered in stroke, but functional significance of this finding has remained unclear. Here, we examined CMC in acute stroke patients and correlated the results with clinical outcome measures and corticospinal tract (CST) integrity estimated with diffusion tensor imaging (DTI). During isometric contraction of the extensor carpi radialis muscle, EMG and magnetoencephalographic oscillatory signals were re…

Combined effect of AAV-U7-induced dystrophin exon skipping and soluble activin Type IIB receptor in mdx mice.

2012

Adeno-associated virus (AAV)-U7-mediated skipping of dystrophin-exon-23 restores dystrophin expression and muscle function in the mdx mouse model of Duchenne muscular dystrophy. Soluble activin receptor IIB (sActRIIB-Fc) inhibits signaling of myostatin and homologous molecules and increases muscle mass and function of wild-type and mdx mice. We hypothesized that combined treatment with AAV-U7 and sActRIIB-Fc may synergistically improve mdx muscle function. Bioactivity of sActRIIB-Fc on skeletal muscle was first demonstrated in wild-type mice. In mdx mice we show that AAV-U7-mediated dystrophin restoration improved specific muscle force and resistance to eccentric contractions when applied a…

Ultrastructure of Dural Lesions Produced in Lumbar Punctures

2014

During lumbar puncture, the insertion of a single-use, short-beveled spinal needle produces a “tent-like” effect. The dura-arachnoid lesions produced by this Quincke type of needles show a “crescent moon” shape resembling the letter “U” or “V” (similar to the lid of a can), with clean-cut edges. As the needle tip advances, the cut fragment is folded inwards, while all dural and arachnoid layers are entirely severed and the edges of the lesion are displaced inwardly. As soon as the spinal needle is withdrawn, the edges of the lesion tend to retract, owing to the viscoelastic properties of the affected dura mater.

Ultrasound-guided musculoskeletal interventional procedures around the elbow, hand and wrist excluding carpal tunnel procedures.

2021

Ultrasound is a fast, reliable and radiation-free method for the assessment of a wide range of pathological conditions, as well as for the guidance of percutaneous interventional procedures around the elbow, hand and wrist. Intraarticular and periarticular interventional procedures can be easily performed under continuous ultrasound monitoring to ensure correct needle positioning and medication delivery to a specific target. The most common ultrasound-guided procedures performed around the elbow, wrist, and hand are described in this review, excluding carpal tunnel procedures. Specifically, elbow steroid injections are performed in patients with inflammatory disorders, while hyaluronic acid…

Kinematics and kinetics analysis of midfoot joints of 30 normal subjects during walking

2016

Abstract Introduction There is not enough evidence in form of clinical studies regarding the behavior of the midfoot joints in dynamic situations such us walking or running. The present work aims to study the mechanical behavior of midfoot joints with a multisegmented foot model with special interest in joint moments and their clinical significance. Subjects and methods A computerized 3-dimensional gait study was performed on healthy male adult subjects with a neutral Foot Posture Index (FPI) (from 0 to +5) during walking. Joint angle and external moments were estimated with a multisegment foot model that considers three separate segments (forefoot, rearfoot and hallux) and graphs and value…

Effect of saddle height on skin temperature measured in different days of cycling

2016

Infrared thermography can be useful to explore the effects of exercise on neuromuscular function. During cycling, it could be used to investigate the effects of saddle height on thermoregulation. The aim of this study was to examine whether different cycling postures, elicited by different knee flexion angles, could influence skin temperature. Furthermore, we also determined whether the reproducibility of thermal measurements in response to cycling differed in the body regions affected or not affected by saddle height. Sixteen cyclists participated in three tests of 45 min of cycling at their individual 50 % peak power output. Each test was performed in a different knee flexion position on …

Positive association of high-density lipoprotein cholesterol with lumbar and femoral neck bone mineral density in postmenopausal women.

2021

Abstract Objective Experimental studies suggest that lipids affect bone metabolism. We aimed to elucidate whether lipid levels are associated with bone mineral density (BMD) in a cohort of postmenopausal women. Design A cross-sectional study of participants in the Chronic Ailment Reduction after MENopause (CARMEN) cohort. Women underwent assessment of clinical and analytical parameters, including fasting lipid levels. BMD was assessed at both lumbar spine and hip. Homogeneity in the cohort was optimized by filtering out a series of confounding variables with a known impact on bone. Main outcome measures Association of BMD at lumbar spine and femoral neck with lipid levels. Results A total o…

Reconstruction of vertebral body in thoracolumbar AO type A post-traumatic fractures by balloon kyphoplasty: a series of 85 patients with a long foll…

2019

Background Traumatic fractures of the thoracolumbar spine are common injuries, accounting for approximately 90% of all spinal traumas. Optimal management of these fractures still gives rises to much debate in the literature. Currently, one of the treatment options in young patients with stable traumatic vertebral fractures is conservative treatment using braces. Kyphoplasty as a minimally invasive procedure has been shown to be effective in stabilizing vertebral body fractures, resulting in immediate pain relief and improved physical function with early return to work activity. The aim of the study is to report VAS, ODI scores, and kyphosis correction following treatment. Methods This is a …

Spinal and supraspinal mechanisms affecting torque development at different joint angles

2015

INTRODUCTION We examined the neural mechanisms responsible for plantar flexion torque changes at different joint positions. METHODS Nine subjects performed maximal voluntary contractions (MVC) at 6 ankle-knee angle combinations [3 ankle angles (dorsiflexion, anatomic position, plantar flexion) and 2 knee angles (flexion, full extension)]. Neural mechanisms were determined by V-wave, H-reflex (at rest and during MVC), and electromyography during MVC (RMS), normalized to the muscle compound action potential (V/Msup, Hmax/Mmax, Hsup Msup and RMS/Msup) and voluntary activation (VA), while muscle function was assessed by doublet amplitude. RESULTS MVC and doublet amplitude were significantly low…

Levetiracetam‐induced pediatric toxic epidermal necrolysis successfully treated with etanercept

2020

Successful management of toxic epidermal necrolysis (TEN) with tumor necrosis factor-α inhibitors has been described in adults, but few cases have been reported in children. To date, only four pediatric cases of TEN treated with infliximab and one with etanercept have been published. We present the case of an 8-year-old boy diagnosed with TEN induced by levetiracetam, successfully treated with etanercept, systemic corticosteroids, and intravenous immunoglobulin.
