Conversion ratios for opioid switching in the treatment of cancer pain: a systematic review

In this paper we describe the results of a systematic search of the literature on conversion ratios during opioid switching. This is part of a project of the European Palliative Care Research Collaboration to update the European Association for Palliative Care recommendations for the use of opioid analgesics in the treatment of cancer pain. Studies were eligible for inclusion if they involved adult patients with chronic cancer pain, contained data on opioid conversion ratios, were prospective and were written in English. Abstract This chapter is focused on opioid dependence treatment and is framed in a holistic perspective. Specifically, there are three main approaches to pharmacological treatment of opioid dependence: opioid detoxification, agonist maintenance, and antagonist maintenance. In last years, heroin-addicted have exponentially increased: this has made it necessary to identify a pharmacological strategy as effective as possible. With this purpose, a statistical investigation was conducted in a sample of individuals, aged between 18 and 50. They were diagnosed and subjected to different treatments at Ser.T of Caltanissetta (Sicily-Italy) during the period 2013-2017. The analyzed patients were treated with three different pharmacological therapy: methadone 0.1%, methadone 0.5%, buprenorphine and suboxone.
