Elevated plasma histamine concentration as a sensitive real-time parameter for distinct phases of surgical trauma: a tool for technology assessment.

1991

High technology plays an important role in surgery, either to expand surgical procedures or to reduce tissue trauma, which is a main goal of minimum invasive surgery. Due to the tremendous increase of costs the problem of technology assessment will not only be solved by statistical methods, but also by biomedical considerations and laboratory tests. Histamine release could be an indication for tissue trauma, which is caused directly by tissue damage or indirectly by infection or hypoxia. Therefore histamine release was investigated during different phases of operation in two clinically very important and complex situations: resection of liver metastases of colorectal carcinoma and resection…

Gastric acid secretory responses induced by peptone are mediated by capsaicin-sensitive sensory afferent neurons

1992

The involvement of capsaicin-sensitive afferent neurons in modulating acid-secretory responses to peptone, a product of protein digestion, has been investigated in the continuously perfused stomach of the urethan-anesthetized rat. Systemic neonatal pretreatment with capsaicin, which destroys primary afferent neurons, does not modify basal levels of acid secretion. Acid responses to intragastric perfusion with isotonic (0.5, 1, and 2.4%) or hypertonic (10 and 20%) solutions of peptone were reduced in capsaicin-treated rats. Intragastric perfusion with hypertonic mannitol (18%) did not stimulate secretion of acid. Systemic capsaicin pretreatment did not modify acid responses to intraperitone…

Effects of Steatosis on Hepatic Hemodynamics in Patients with Metabolic Syndrome

2015

The aim of our study was to assess the hemodynamic changes in hepatic and splenic circulation using B-mode ultrasonography and color Doppler ultrasonography, in a population of patients with metabolic syndrome divided with respect to the presence or absence of steatosis diagnosed by ultrasonography. One hundred forty-one patients were included in the study. The severity of non-alcoholic fatty liver disease was classified as mild, moderate or severe. Visceral fat thickness, longitudinal diameter of the spleen, diameter of the portal vein, mean maximum portal vein flow velocity, hepatic artery and splenic artery resistivity indexes and hepatic vein flow phasicity were measured. Non-alcoholic …

Refractive changes in nuclear, cortical and posterior subcapsular cataracts. Effect of the type and grade

2015

Purpose: To determine the effect of main morphological types and grades of age-related cataracts on refractive error. Methods: We measured 276 subjects with optical compensation prior to the development of cataract. We evaluated 224 eyes with nuclear cataract, 125 with cortical cataract, and 103 with posterior subcapsular (PSC) cataract classified with LOCSIII. We measured visual acuity (VA) with their spectacles and best-corrected visual acuity (BCVA) with chart in decimal scale to obtain the optimal compensation with cataract. We evaluated the differences between compensations. Results: A significant myopic shift was observed in nuclear cataract from low to mild grade (p = 0.031), the sam…

Relationship between adherence to the Mediterranean Diet, intracerebral hemorrhage, and its location

2018

INTRODUCTION: Although some authors evaluated the relationship between adherence to the Mediterranean Diet (MeDi) and both ischemic and hemorrhagic stroke, hemorrhagic stroke alone is not yet examined. AIMS: We conducted a retrospective study to evaluate the relationship between adherence to MeDi and intracerebral hemorrhage (ICH) and different locations of ICH (ganglionic/internal capsule, brainstem/cerebellum, or lobar). METHODS: We analyzed charts and collected data of all consecutive patients with ICH admitted to our Internal Medicine Ward from 2005 to 2014. A scale indicating the degree of adherence to the traditional MeDi Score was constructed. RESULTS: When compared with 100 subjects…

CT after enucleation of renal cell carcinoma.

1994

Computed tomography (CT) was performed in 28 patients 2 weeks to 120 months after enucleation of renal cell carcinoma. The postoperative defect could be exactly localized in all patients. A wedge-shaped (N = 11) or concave (N = 9) morphology was typical. No dependence between morphology of the defect and localization or size of the tumor or the operative technique was observed. The defects were smaller (1.9 cm) than the original prominent tumors (3.6 cm). Defect size was dependent on the operative technique: closure by suture of the renal capsule in smaller defects or by fat flap in larger ones. Postoperative hematomas or delayed perfusion in the adjacent parenchyma were seen in five patien…

Effect of large positioning holes on capsule fixation of plate-haptic intraocular lenses

2000

Abstract Purpose To compare the centration and fixation of silicone plate-haptic intraocular lenses (IOLs) with different-sized positioning holes. Setting Eye Clinic of the Johannes Gutenberg-University Mainz, Mainz, Germany. Methods In a prospective randomized study, 51 Chiroflex C10 and 56 Chiroflex C11 IOLs were implanted under standardized conditions by the same surgeon. The IOL position was documented at the end of surgery and by retroillumination on the first day and 5 months postoperatively. The positioning-hole area was evaluated by ultrasound biomicroscopy (50 MHz) 5 months postoperatively. Results One day postoperatively, no IOL in either group was decentered more than 1.0 mm. Aft…

Capsule measuring ring to predict capsular bag diameter and follow its course after foldable intraocular lens implantation

2003

Purpose: To evaluate the extent of capsular bag shrinkage after cataract surgery with intraocular lens (IOL) implantation and develop a regression formula to predict postoperative capsular bag size. Setting: Eye Hospital, Johannes Gutenberg-University, Mainz, Germany. Methods: The axial length (AL), anterior chamber depth, and corneal radius in 58 eyes were measured preoperatively. Cataract surgery was by phacoemulsification followed by implantation of a 3-piece, acrylic, posterior chamber IOL. The capsular bag diameter and anterior capsulorhexis were measured intraoperatively and 1 day and 1, 3, and 6 months postoperatively using a Koch capsule measuring ring (HumanOptics). Results: The me…

Effect of a closed foldable equator ring on capsular bag shrinkage in cataract surgery.

2005

To evaluate the effect of a closed foldable equator ring (CFER) versus a conventional capsular tension ring (CTR) on capsular bag shrinkage.Department of Ophthalmology, Johannes Gutenberg-University, Mainz, Germany. In this prospective study, 70 eyes of 70 patients were randomized to 2 groups using a 1:1 block scheme. After uneventful cataract surgery, a capsular measuring ring was implanted in all eyes to measure the capsular bag diameter in vivo. In Group 1, a CTR was implanted in the capsular bag. In Group 2, a CFER was inserted. Biometric characteristics such as axial length and the horizontal and vertical corneal radii were measured preoperatively. The capsular bag diameter and capsulor…
