Idarucizumab in High-risk Thoracic Surgery

Direct oral anticoagulants have suggested a favorable profile compared with vitamin K antagonists. However, the lack of treatment to reverse the effect of direct oral anticoagulants has limited its use in some patients who require rapid reversal of anticoagulation, as those included in the transplant waiting list. Idarucizumab is a recently approved drug to reverse the anticoagulant effect of dabigatran. However, the clinical experience when using this drug is scarce. Herein, we present a clinical case on anticoagulation reversal with idarucizumab to perform heart and lung transplantation in a patient with Eisenmenger syndrome.

This disease has a spectrum of different clinical pictures with different outcomes. Herein, we report all the data from three paucisymptomatic patients during a hospital stay that might represent a paradigmatic example of the method by which SARS-CoV-2 is shed. We demonstrated the lack of an adequate qualitative and quantitative immune response by multiparametric flow cytometry analysis. Iatrogenic lumbar artery pseudoaneurysm after lumbar transpedicular fixation: Case report

Only a few cases of iatrogenic lumbar injury have been reported in the literature. A 58-year-old male with severe back pain was admitted to our department. The patient was qualified for the transpedicular stabilization with interbody fixation at L4/L5 and L5/S1 levels. On the first day after surgery, the patient was mobile and reported significant pain relief. He was discharged the same day. After 2 weeks, the patient returned with severe right lower abdominal pain and radicular pain in his right leg. A 16-month-old boy was referred to the emergency department of the Children's Clinical University Hospital (Riga, Latvia) due to cough and noisy breathing for 3 months. The complaints seemed to have worsened over time with coughing fits 2–4 days a week and the boy not being able to tolerate solid food (leading to vomiting) during the fits. In between the bouts, he felt fine. Diminished food intake was noted over the last month. Over the 3-month period, no other symptoms were noted. A 65-year-old man was referred for evaluation of acute onset of fever, productive cough and dyspnoea. He had previously received a diagnosis of laryngeal carcinoma, which had been treated with laryngectomy and bilateral laterocervical lymphadenectomy, followed by chemotherapy. This preliminary study aimed to investigate the effects of a six-week low-frequency electromyostimulation training (10Hz) on the cardiovascular, respiratory and muscular systems. To that purpose, aerobic capacity, knee extensor muscles strength and architecture, muscle sympathetic nervous activity, blood pressure and heart rate have been evaluated in one healthy male subject (33 year-old, 1.73 m, 73 kg). Results showed improvement of aerobic capacity (+4.5% and +11.5% for maximal oxygen uptake and ventilatory threshold) and muscle strength (+11% and +16% for voluntary and evoked force). We report the case of a 59-year-old male patient suffering from locked-in syndrome (LIS) following basilar artery thrombosis despite an attempt of thrombolysis. Neurological examination showed quadriplegia and aphonia and a state of coma requiring mechanical ventilation was diagnosed. The use of 18F-fluorodeoxyglucose (18F-FDG)-positron emission tomography (PET) allowed to detect a normal 18F-FDG uptake in the main cerebral cortical areas and a significant reduction of 18F-FDG uptake in both cerebellar hemispheres, compatible with a functional deafferentation, helping confirming the clinical suspicion of LIS. Seizure following the Use of the COX-2 Inhibitor Etoricoxib

We describe a case of epileptic seizures occurring after the use of a COX-2 inhibitor. A 61-year-old man was admitted to our department because of a generalized tonic-clonic seizure. EEG showed generalized slowdown of the activity. Neuroimaging and blood samples studies did not evidence alterations, but a careful pharmacological history revealed that the patient had taken the COX-2 inhibitor etoricoxib to treat lumbago few days before the onset of clinical symptoms. No seizures were reported after etoricoxib discontinuation and an EEG resulted to be normal two months after this. Conclusion.
