Transfusion Therapy in a Multi-Ethnic Sickle Cell Population Real-World Practice. a Preliminary Data Analysis of Multicentre Survey

2018

Abstract Introduction. Despite the increasing of number of patients with Sickle Cell Disease (SCD) in Italy, due to multi-ethnic migratory phenomena, a large percentage of Caucasian sickle population is already present in Italy mainly with b-thal/HbS genotype. Red cell transfusion is one effective treatment for both acute and chronic complications of SCD, while hydroxycarbamide (HC) is used to reduce the frequency of painful vaso-occlusive crises (VOCs) and decrease the need for blood transfusion. Through the National Comprehensive Reference Centers for SCD, the Italian Society of Thalassemia and Hemoglobinopathies (SITE), in collaboration with the Society Italian Transfusion Medicine and I…

SIRM–SIC appropriateness criteria for the use of Cardiac Computed Tomography. Part 1: Congenital heart diseases, primary prevention, risk assessment …

2021

AbstractIn the past 20 years, Cardiac Computed Tomography (CCT) has become a pivotal technique for the noninvasive diagnostic work-up of coronary and cardiac diseases. Continuous technical and methodological improvements, combined with fast growing scientific evidence, have progressively expanded the clinical role of CCT. Recent large multicenter randomized clinical trials documented the high prognostic value of CCT and its capability to increase the cost-effectiveness of the management of patients with suspected CAD. In the meantime, CCT, initially perceived as a simple non-invasive technique for studying coronary anatomy, has transformed into a multiparametric “one-stop-shop” approach abl…

2016

Tutkimus keskittyy vuosien 1918 - 1919 espanjantautina tunnettuun influenssapandemiaan Iso-Britanniassa kolmen aineistokokonaisuuden: sanomalehti The Manchester Guardianin, lääketieteellisen The British Medical Journal -lehden ja Iso-Britannian parlamentissa käytyjen keskusteluiden kautta. Tarkastelun kohteena on influenssasta poliittisella, professionaalisella, julkisella ja paikallisella tasolla käytyjen keskusteluiden luonne, niiden teemat ja keskusteluissa tapahtuneet muutokset kolmen eri influenssa-aallon aikana, sekä eri tasoilla käytyjen keskusteluiden mahdolliset erot ja yhtäläisyydet. Tutkimus havainnollistaa influenssapandemian yhteiskunnallisten vaikutusten laaja-alaisuutta aina …

Coronary artery fistulas: symptoms may not correlate to size. An emblematic case and literature review

2015

Coronary artery fistulas are rare anatomic abnormalities of the coronary arteries present in 0.002% of the general population and represent 14% of all anomalies of coronary arteries. Their clinical relevance focuses mainly on the mechanism of "coronary steal phenomenon”, causing myocardial functional ischemia even in the absence of stenosis, hence common symptoms are angina or dyspnea. Small size fistulas are mostly asymptomatic and have excellent prognosis if managed medically with regular follow-up consisting also in echocardiography every 2-5 years. Big-sized and symptomatic fistulas, on the contrary, should undergo invasive closure, either with a transcatheter approach or with …

Amazon's initiative transforming a non-contact society : Digital disruption leads the way to stakeholder capitalization

2021

Contrary to the decisive role of R&D centered on information and communication technology (ICT) in the digital economy, its excessive expansion has resulted in declining productivity due to the two-faced nature of ICT. Consequently, the novel concept emerges of innovation that maintains sustainable growth by harnessing the vigor of soft innovation resources (SIRs). Pioneering endeavors can be observed at the forefront of the global ICT leaders. World R&D leader Amazon has been harnessing the power of users that seek SIRs. This functions as a virtuous cycle, leading to the transformation of R&D by fusing a unique R&D system with a sophisticated financing system. With this orchestration, Amaz…

Dissolution and hydrolysis of fibre sludge using hydroxyalkylimidazolium hydrogensulphate ionic liquids

2014

The dissolution and hydrolysis of wet fibre sludge in ionic liquids (ILs) with different reaction conditions are performed in this study. Novel types of hydroxyalkylimidazolium hydrogensulphate ILs, [glymim]HSO4, [hemim]HSO4 and [hpmim]HSO4, are especially designed and aimed to combine the dissolution and hydrolysis of wet fibre sludge in a one-step pretreatment. The results were obtained based on the analysis of total reducing sugars (TRS) with the DNS method. The dissolution and hydrolysis of fresh wild horse chestnut fruit (Aesculus hippocastanum) with the same ILs were also carried out as a comparison reference. Since fibre sludge is cellulose based and horse chestnut fruit is starch ba…

Management of Pulmonary Embolism: Results from the German Chest Pain Unit Registry.

2020

Introduction: Since 2008, specialized chest pain units (CPUs) were implemented across Germany ensuring structured diagnostics in acute chest pain. This study aims to analyze the management of pulmonary embolism (PE) patients in such certified CPUs. Methods: Data were retrieved from 13,902 patients enrolled in the German CPU registry and analyzed for the diagnosis of PE including patient characteristics, critical time intervals, diagnostic workup, treatment, and prognosis. PE patients were compared to the overall CPU patient cohort. Only patients with a complete 3-month follow-up were included. Resu…

Diagnostic and prognostic value of H-FABP in acute coronary syndrome: Still evidence to bring

2018

Abstract The assessment of chest pain patients presenting to the emergency area (EA) is still a clinical challenge, as the majority of patients are not diagnosed with acute coronary syndrome (ACS). New generation high sensitivity c-Tn (hs-cTn) assays have showed better performances compared to the standard c-Tn. However, hs-Tn still presents some limitations. Hence, novel, early biomarkers are needed in this setting. Among all, heart-type fatty acid binding protein (H-FABP) has been largely investigated. This article reviews the studies evaluating H-FABP performance in diagnosing acute myocardial infarction (AMI) and stratifying chest pain patients by risk. H-FABP optimal performances in AC…

C-Reactive Protein in Acute Coronary Syndrome. Looking Back in Order to Move Forward

2006

Recent years have seen a spectacular rise in the importance of biomarkers in acute coronary syndrome (ACS). The most notable of these biomarkers is, without doubt, troponin. Its usefulness for diagnosis, decision making, and prognostic stratification have been fully validated, and its use in daily clinical practice is now widespread.1 It should be pointed out that the kinetics of troponin release were studied in detail for different types of ACS right from the outset. This approach has clearly laid a solid foundation which has contributed to the current popularity of this marker. However, it was soon evident that not all patients with ACS and elevated troponin had a very poor prognosis,2-6 …
