Dehn surgeries and smooth structures on 3-dimensional transitive Anosov flows.

2020

The present thesis is about Dehn surgeries and smooth structures associated with transitive Anosov flows in dimension three. Anosov flows constitute a very important class of dynamical systems, because of its persistent chaotic behaviour, as well as for its rich interaction with the topology of the ambient space. Even if a lot is known about the dynamical and ergodic properties of these systems, there is not a clear understanding about how to classify its different orbital equivalence classes. Until now, the biggest progress has been done in dimension three, where there is a family of techniques intended for the construction of Anosov flows called surgeries.During the realization of this th…

Macular sensitivity and structure in idiopathic epiretinal membranes 12 months after surgical treatment

2013

Purpose: To evaluate macular sensitivity (MS) change and its correlation with visual acuity (VA) and Spectral-Domain Optical Coherence Tomography (SD-OCT) in patients with idiopathic epiretinal membrane (ERM) at 3, 6 and 12 months after surgical treatment. Methods: Prospective, monocenter, interventional case series. We included 49 patients (49 eyes) with an idiopathic ERM scheduled for a surgical treatment. Among these patients, 40 were evaluated at 3 and 6 months (M3, M6), and 31 at 12 months (M12) after ERM and internal limiting membrane removal. At each visit a measurement of best-corrected visual acuity (ETDRS), a microperimetry and a SD-OCT were performed for all patients. Results: The …

The influence of surgical experience on the rate of intraoperative aneurysm repture and its impact on aneurysm treatment outcome.

2001

Abstract BACKGROUND The influence of surgical experience on the result of aneurysm surgery remains unclear. To determine the impact of surgical experience we considered the occurrence of intraoperative aneurysm rupture (IAR) during microneurosurgery for intracranial aneurysms as an objective factor that could be evaluated. METHODS A retrospective study was performed on 379 consecutive patients with 490 cerebral aneurysms operated upon from 1989 to 1995. RESULTS IAR occurred in 6.7% of aneurysms and 8.7% of patients. There was a direct inverse relationship between the annual caseload of the surgeon and the risk of IAR. New neurological deficits (NND) occurred in 21% of patients with IAR, whi…

Thinking about movement hurts: The effect of motor imagery on pain and swelling in people with chronic arm pain

2008

Objective: Chronic painful disease is associated with pain on movement, which is presumed to be caused by noxious stimulation. We investigated whether motor imagery, in the absence of movement, increases symptoms in patients with chronic arm pain. Methods: Thirty‐seven subjects performed a motor imagery task. Pain and swelling were measured before, after, and 60 minutes after the task. Electromyography findings verified no muscle activity. Patients with complex regional pain syndrome (CRPS) were compared with those with non‐CRPS pain. Secondary variables from clinical, psychophysical, and cognitive domains were related to change in symptoms using linear regression. Results: Motor imagery in…

CT-Arthrographie der Schulter nach operativer Versorgung ventraler Instabilitäten

1991

25 patients who had undergone a modified Lange repair were examined postoperatively after about four years, both clinically and by CT arthrography. Ventral redislocation was no longer seen postoperatively. As regards residual ventral instability, there was agreement between CT arthrography and the clinical findings in 20/25 patients (90%). Newly developed or residual dorsal instability could be accurately demonstrated by the presence of pathological changes of the dorsal labrum and by the observation of a wide dorsal joint recess. CT arthrography did not provide reliable evidence concerning loss of function and mobility of the treated shoulder. However, the presence of marked capsular foldi…

Information asymmetries and supplier induced demand (an economic study for the french market)

1990

The supplier induced demand hypothesis relies on the existence of information asymmetry between the physician and his patient. This asymmetry confers a discretionary power on practitioners who are using it in order to satisfy their own interest. The inducement effect is identified and distinguished from the other consequences of information asymmetries in the health care system, i.e. moral hazard and adverse selection. A controversy is generated by the induced demand hypothesis which constitutes a challenge to the assumption of the independence of supply and demand and, therefore, contradicts standard neoclassical predictions. The role of empirical studies is important in this respect. An e…

Design of molecular imaging and theranostic agents for the detection and targeted therapy of cancers

2020

Despite significant advances in cancer detection and treatment, cancer is now the second leading cause of death worldwide. The aim of this thesis project was to design and optimize imaging agents for the diagnosis and/or therapy of various cancers. The first axis of this thesis focused on the development of a bimodal PET/fluorescence imaging agent capable of targeting neurotensin NTS1 receptors overexpressed in certain cancers. PET imaging would allow efficient detection of tumors and their metastases, while fluorescence imaging would facilitate the evaluation of tumor margins during surgery. Several compounds were synthesized, labelled with gallium-68 and studied in preclinical studies in a…

Hip muscle function in patients with symptomatic femoroacetabular impingement

2014

Femoroacetabular impingement (FAI) is a pathomechanical process of the hip joint, which could lead to hip pain and functional disability. Aim of this thesis was to investigate hip muscle function in patients with a symptomatic FAI. Hip muscle function was first investigated before patients underwent any surgical treatment for managing FAI. It was shown that they present with reduced hip muscle strength (i.e., muscle weakness), probably due to hip muscle inhibition. Nevertheless, hip muscle weakness was not associated with exaggerated hip muscle fatigue. Hip muscle strength recovery was then evaluated in a series of patients after hip arthroscopy to treat FAI. These patients demonstrated a g…
