Carl Florian Toldt was an Austrian anatomist who made meaningful contributions worldwide and defined what is one of the most important surgical landmarks in abdominal surgery. Computed tomography of bowel obstruction: Tricks of the trade

Bowel obstruction (BO) is a frequent cause of hospitalization and surgical consultation for acute abdominal pain. It is usually suspected at physical examination, but clinical and laboratory data are often nonspecific. Thus, computed tomography plays a crucial role in a correct diagnosis of BO. Indeed, computed tomography can confirm a diagnosis of BO, and identify the location and cause of the obstruction. In this review, the computed tomography appearances of common and uncommon causes of BO and pseudo-obstruction are reviewed.

The clinical relevance of non-classified dysganglionoses and implications for a new grading system

In addition to the classified types of dysganglionosis, certain non-classified dysganglionoses (NCD) (types 1-4) were introduced by Meier-Ruge in 1992. Clinical data on these conditions are limited. Among 134 children with intestinal dysganglionoses (ID) treated from 1979 to 1999, 12 were identified to have a NCD. Their clinical course is presented. The existence of mild ID (type 1) is difficult to demonstrate. Current definitions and data on clinical relevance are not convincing. An indication for surgical treatment is not present. A rat colon cancer cell line (CC531) was implanted into the livers of 85 WAG rats and in animals surviving the implantation the liver tumour was subsequently treated with sham laparotomy (n = 15), resection (n = 20), cryotherapy (n = 19) or laser therapy (n = 20). Local recurrence or persistence of the tumour in animals surviving the procedure was observed in 15 of 15 following sham laparotomy, in 0 of 15 following resection, in 3 of 17 following cryotherapy and in 10 of 14 following laser therapy. However, peritoneal disease and lung metastases were more frequent and extensive following resection as compared to cryotherapy or laser therapy.

What should an ideal screening and surveillance colonoscopy be able to accomplish? The technique should allow detection of large but also discrete mucosal alterations. Training program for transanal endoscopic microsurgery.

Televised endoscopy and the concept of the “assisted” endoscopic operation is of great help in teaching surgical endoscopic techniques. The use of training dummies provides a new method of training manual dexterity and surgical skills in special courses or in surgical skill laboratories. We have developed a training system for transanal endoscopic microsurgery. Operations with our technique were performed on 116 patients.
